Job summary

Clayton Medical Centre is seeking a part time Practice Nurse to join our friendly, family Practice to deliver high quality nursing care.

Clayton Medical Centre Serves around 6,000 patients in and around Clayton-le-Moors. We pride ourselves on being a friendly welcoming team, providing a good quality, caring services to our patients and a range of standard NHS and local enhanced services.

Main duties of the job

Responsiblities will include chronic disease management, including COPD, Asthma, Diabetes and Coronary Heart Disease, cervical screening, childhood vaccinations, delivery of local enhanced services and health promotion.

Ideally you will have a knowledge and understanding of Travel health, dressings and contraceptive services.

You will also be responsible for providing the above services to housebound individuals and therefore need a full UK driving licence, access to your own vehicle and appropriate insurance.

You will need good IT skills in order to maintain appropriate medical records using data entry templates and participate in audits and data collection as necessary.

About us

The Practice team currently consists of: 3 GP Partners, 3 Salaried GP, 1 Advanced Nurse Practitioner, 2 Practice Nurses, 1 Assistant Practitioner and 2 Healthcare Assistants. Supporting our Clinical Team are our Senior Administrator, Reception Supervisor, 4 care navigators, 2 Administrators and 1 Prescription Administrator. We are a training practice and regularly welcome FY2, GPST and student nurses as part of their training rotation.

  • Support the Infection Control Lead and work to the IPC policy.
  • Undertake Holistic patient assessments with patient and their families.
  • Support long term condition management including involvement with quality improvement and assurance initiatives. Teaching patients and families how to manage their condition.
  • Support the management of care for frail and housebound patients.
  • Support public health screening programmes including immunisation, vaccination, cervical screening and provide opportunistic health promotion advice.
  • Support population health management targeting health inequalities and improving access.
  • Provide routine nursing care in accordance with clinical based evidence, NICE and NSF.
  • Participate in clinical audit and research projects including supporting development and updating of practice protocols and guidelines.
  • Work within national and local protocols.
  • Recognise boundaries of practice and know when and to whom patients should be referred.
  • Support delivery of enhanced services and other service requirements.
