newly qualified GP to join our PCN as part of the Additional Roles
Reimbursement Scheme (ARRS). This is an exciting opportunity to be part of an

which covers 4 practices, Moorside, Ashcroft, Rockwell and Wrose and Farrow in

This is a fantastic opportunity to experience the diversity
of primary care within the 5LE PCN, working on a fulltime basis.
This would be on a fixed term basis, but with a view to a
permanent role once NHS England have confirmed funding for these ARRS roles.The role may require you to work within our Enhanced Access
set up on evenings and weekends and therefore a level of flexibility is
required.
Main duties of the job
All general duties and responsibilities associated with a GP
working within primary care:

Undertaking a variety of duties including
surgery face to face consultations, telephone consultations and queries,
visiting patients at home, prescription administration and management, general administration (referrals, pathology,reports,
correspondence) in a timely fashion, ensuring high quality care.

Making professional, autonomous decisions in
relation to presenting medical problems, whether self-referred or referred from
other healthcare workers within the organisation.

Assessing the healthcare needs of patients with
undifferentiated and undiagnosed problems.
-
Screening patients for disease risk factors and
early signs of illness.
-
Developing care plans for health in consultation
with patients and in line with current practice disease management protocols.

Providing health education.

Contribute to optimise PCN Investment and Impact
Fund ( quality improvement markers)

Recording clear consultation notes to agreed
standards.

Collecting data for audit purposes and complying
with QOF requirements.

About us
We are the 5LE PCN based in Bradford. The PCN covers 4 practices, which include Farrow Medical Practice, Moorside Surgery, Ashcroft Surgery and Rockwell and Wrose Practice. Our PCN population covers a total of 36,336.55 patients.
The 5LE PCN is known for their friendly, forward-thinking approach in committing to delivering high quality care for our 36000+ patients. We also pride ourselves in providing a safe environment for our staff by providing a dedicated PCN team, which include a PCN Network manager, clinical supervisor, care coordinators and ACPs. The PCN employees a dedicated PACTeam (Proactive Care Team) and a HBVTeam (Housebound Visiting Team) who provide support for our housebound patients. The PCN team also provide support to the practices by subcontracting FCPs, pharmacists and Social Prescribers.

at home, checking and signing/authorising repeat prescriptions and dealing with
