Job summary

Our successful, high achieving and innovative Surgery with a ANP/GP Joint Partnership, is offering you the opportunity to work with us. We are looking for an experienced Practice Nurse to join us working in a busy general practice as part of our dedicated clinical team along with friendly and supportive clinical and administrative staff. We have approximately 4600 patients.

Main duties of the job

The Post Holder is a Practice Nurse who, acting within their professional boundaries within the NMC standards. You will perform routine tasks related to patient care as directed by the Clinicians; Cytology Health Checks, Health Promotion, ECGS, Asthma, Peak Flow Readings, Spirometry, Chronic disease management including diabetes, respiratory disease, CVD, Routine immunisations, childhood immunisations, chaperoning and assist patients where appropriate who are being examined by another clinicians. Actively participate in meeting national (QoF) and locum contract targets (IIF/KLOE/DES).

They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

Demonstrate critical thinking in the clinical decision-making process.

They will work collaboratively with the general practice team

Family Planning

Following agreed clinical protocols

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.

Proactively identify, diagnose and manage treatment plans

Diagnose and manage both acute and chronic conditions

Review medication

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

Other Tasks will include:

Clean and sterilise equipment

Ordering of stock

Ordering of vaccinations to maintain stock levels

Participation in administrative systems in the Practice

At all times there will be a need to maintain accurate records as per NMC guidelines.

A duty to advise senior clinicians of potential problems or errors within the range of assigned tasks

Attend and participate in all Practice Meetings when required

To participate in developments within the practice and support QoF requirements.
