Job summary
SHaR PCN is a group of seven doctor's surgeries and medical centres that work closely together as part of a network. It employs health professionals including clinical pharmacists, occupational therapists, first contact physiotherapists, paramedics, urgent care practitioners, health and wellbeing coaches, first contact mental health practitioners, specialist care co-ordinators (e.g. dementia and frailty) and social prescribing link workers. Working as part of multi-disciplinary teams, these clinical and non-clinical health professionals provide tailored care for patients allowing GPs to focus more on patients with complex needs.
Whilst working for and across the SHaR PCN network, the host practice that will manage the role on a day to day basis is Pickering Medical Practice.
Main duties of the job
High quality data collection, co-ordination and monitoring is essential in focussing and improving the quality of care to patients and tackling health inequalities.
The role will provide data reports, data quality and assurance support to the South Hambleton and Ryedale Primary Care network (SHaR PCN) through monitoring the accuracy of the data in the clinical systems as well as providing audit support and activity reporting.
The role will also include supporting digital initiatives and responsibilities for the co-ordination of patient journeys through primary care with a particular emphasis on the digital journey.

The duties and responsibilities to be undertaken by the Senior Data Coordinator may include any or all of the items in the following list:
Liaising with PCN practices to ensure coding is compliant and consistent.
Running system reports and extracting data as required by the PCN for activity and demand reporting, extracting and collating data as required for the PCN.
Promoting digital inclusion and to reduce digital exclusion
Implementing data quality assurance on all practice activity data
Identifying patient cohorts requiring additional medical/social support and working with them to support their care requirement.
Providing data administration support to the PCN management team as required.
Gathering information for and arranging and providing administrative support to MDT meetings
Utilise population health intelligence data to proactively identify and work with a cohort of patients so that personalised care can be delivered by the team
Design efficient templates, protocols, alerts and other IT tools to support safety, patient care, quality record-keeping, reporting and financial claims
Carry out audits to identify improvements and maximise income in particular in IIF (Investment and Impact Fund), Enhanced Services and claiming procedures.
