An exciting opportunity has arisen for a Practice Nurse to
join our team at Dartmouth Medical Practice. This is a permanent position for 22-30 hours per week. We are a friendly and forward-thinking practice located
in beautiful South Hams. We are excited to be recruiting for a Practice
Nurse to support our dedicated, well-established
clinical team. This is a fantastic opportunity to contribute to and help shape
a progressive practice with a strong focus on teamwork, innovation and patient-centred
care.

Main duties of the job
Key Responsibilities
Chronic disease management (e.g. diabetes, asthma, COPD, hypertension, cardiovascular disease)
Cervical cytology
Childhood and adult immunisations
Travel vaccinations (where appropriate)
Wound care and dressings
Health promotion and screening
Medication reviews and prescribing (for qualified Nurse Prescribers)

The Practice Nurse will:
Deliver general practice nursing services including:
Wound care and dressings
Cervical cytology
Childhood and adult immunisations
Travel health advice and vaccinations
ECGs
Phlebotomy
Blood pressure monitoring
NHS Health Checks
Undertake chronic disease management (as appropriate to competency), including:
Diabetes
Asthma
COPD
Hypertension
CHD
Provide contraceptive advice and sexual health services (with training)
Support screening programmes (e.g. cervical, bowel, NHS Health Checks)
Participate in infection prevention and control measures
Undertake home visits when clinically appropriate
Maintain accurate, contemporaneous clinical records in line with professional standards
