Job summary

The Practice Nurse will undertake a range of nursing assessments and provide appropriate care and treatment in conjunction with GPs, Nurse Practitioners, and other practice nurses or independently, in line with competence and need. Responsibilities include:

  • Maintaining and improving standards of care, identifying patient population needs.
  • Applying knowledge related to the Quality and Outcomes Framework (QOF).
  • Delivering therapeutic interventions within a broader health promotion/public health strategy.
  • Providing education and training on chronic clinical conditions.
  • Offering specialist advice to patients and relatives regarding COPD, Asthma, and, where qualified, Diabetes.
  • Engaging in further training for role development.
  • Practicing within the NMC Code of Professional Conduct and maintaining accurate records.

Main duties of the job

Main Responsibilities

  • Meet patients nursing care needs, including occasional home visits.
  • Initiate and maintain health screening and promotion clinics.
  • Manage chronic disease care using personal recall systems and practice computer systems.
  • Provide advice and support to primary healthcare professionals.
  • Participate in training, facilitation, and clinical supervision of other practice nurses.

About us

The Neaman Practice, located in the Barbican, is seeking an experienced Practice Nurse to lead our busy and friendly practice. This permanent role requires a commitment of 37.5 hours per week.

We are searching for a candidate who possesses a deep passion for General Practice and is motivated to tackle the challenges presented by the NHS. The ideal candidate will demonstrate exceptional leadership skills and an ability to guide our team in delivering high-quality, compassionate care to our valued patient base, which consists of approximately 10,500 individuals.

Our practice boasts a total staff of around 30 professionals, including salaried GPs, practice nurses, nursing associates, healthcare assistants, pharmacists, pharmacy technicians, a social prescriber, and a large support staff.We take great pride in our practice's history of supporting our staff through various opportunities and training programs, and we are committed to providing excellent career development prospects.
