Job summary

Kiltearn Medical Centre is seeking a dedicated and compassionate Social Prescribing Link Worker (SPLW) to join our team. This role is crucial in supporting patients to improve their health and wellbeing by connecting them with community services and support. The ideal candidate will work collaboratively with patients, healthcare professionals, and local organisations to provide holistic care and support.

Main duties of the job

Manage a caseload of social prescribing referrals.

Conduct one-to-one consultations with patients, focusing on their individual needs and priorities.

Develop personalised support plans in collaboration with patients.

Connect patients with appropriate community services and support groups.

Monitor and support patients' progress towards achieving their health and wellbeing goals.

Liaise with referring health professionals and provide feedback on patient progress.

Promote social prescribing within the Practice and local community.

Build strong relationships with local voluntary and community sector organisations.

Identify gaps in community provision and work with partners to address unmet needs.

Maintain accurate records and provide reports as required.

Ensure compliance with data protection legislation and information governance policies.

Participate in training and development activities to enhance skills and knowledge.

About us

Our Practice is within a purpose-built Primary Care Centre in the heart of Nantwich with 20 clinical rooms and 1 operating suite on the ground floor from which with provide a minor surgery provision and an onsite Research Team.

Nantwich is a charming historical market town, set beside the river Weaver, with excellent transport links with Manchester only an hour away by train, the beautiful city of Chester is a 45-minute drive away and the North Wales coast just over an hour away. Nantwich also has a number of excellent primary schools and two very reputable senior schools.

We serve a population of circa 13,000 patients and our medical team comprises of 6 GP Partners, 2 salaried GPs, 3 Advanced Nurse Practitioners, 1 Nurse Practitioner, 3 Practice Nurses, and 1 Health Care Support Worker and 2 Physiotherapists. We are part of Nantwich and Rural PCN and currently have ARRS roles in Care Co-ordinator, Nurse Associate, 2 Pharmacy Technicians, 2 Pharmacists and a social prescriber.

We also have an onsite dedicated Research Team with 3 Nurses, Admin and three of our own GP Partners, with valuable research studies constantly ongoing there is always the opportunity to become involved in our research team.

Kiltearn is also a GP training practice for F1, F2, ST Doctors and medical students. We also have Partners with an interest in coach mentoring, career conversation facilitating and as appraisers, one of our GP Partners is also the Education Lead for the PCN.

Qualifications

Essential

  • Evidence of literacy and numeracy gained at GCSE or higher.
  • Experience of working in the area of community development/ health and social care, learning support or public health/health improvement;
  • Experience of working to support vulnerable adults, those with mental health issues or experiencing temporary challenges;
  • Partnership/collaborative working and building relationships across a variety of organisations;
  • Excellent written and verbal communication skills
  • Ability to prioritise workload effectively and manage caseload pro-actively and disengage with patient at end of treatment.
  • Competent in use of standard NHS IT packages eg Word, Powerpoint, Excel etc;

Desirable

  • Level 3 in Social Prescribing
  • Experience gained within a PCN or General Practice environment;
  • Experience of working within a social prescribing or equivalent context;
  • Experience of working with the VCSE sector (in a paid or unpaid capacity), including with volunteers and small community groups
  • Motivational coaching and interview skills
  • Ability to produce reports on caseload activities, such as tracking engagement with positive activities per month.
  • Ability to help organise Social Prescribing information event for Tudor Surgery Patients
  • Knowledge of EMIS clinical system
