Main Responsibilities
Administrative Safeguarding Duties:
1. To oversee the workflow of safeguarding reports and documents and ensure they are circulated to appropriate clinicians in line with practice safeguarding policies.
2. To summarise and code the documents into the relevant specific templates, highlighting to patients registered GPs anything significant.
3. To maintain a register of children on Child Protection Plans, Child in Need plans, and those children deemed cause for concern.
4. To ensure the correct coding is added to patient records to aid clinicians in identifying those children and adults who are cause for concern.
5. To add alerts to parent records, those who have a child on a safeguarding plan.
6. To link in with practices and ensure that the training is progressing as anticipated.
7. Any other tasks delegated by practices that are suitable and relevant.
Safeguarding Partner Communications:
1. To prioritise and respond to strategy meeting invitations as advised by the practice safeguarding lead.
2. To respond to child protection meetings as advised by practice safeguarding lead, preparing appropriate documentation and sending apologies as necessary.
3. To monitor DNAs for children in regard to missed clinical appointments and refer to appropriate clinician for follow up (e.g Practice Nurse, usual GP, Safeguarding Lead) in line with practice safeguarding policies.
4. To follow up on those children who have not had their routine immunisations, as identified by administration with the appropriate clinician.(e.g Practice Nurse) in line with practice safeguarding policies.
5. To support the safeguarding lead in co-ordinating communication and meetings with third parties e.g Health Visitors, Mash, CCG Safeguarding Leads and Social Workers. This includes taking minutes and updating the patient record in accordance with agreed actions.
6. To redact and update the EMIS record as per practice safeguarding protocols when a patient reaches 18 years old.
7. To update alerts and warnings in the EMIS record according to practice safeguarding protocols when safeguarding concerns are recorded.
Communications and Working Relationships
Personal Development
1. To attend mandatory training and fire lectures regularly and assist with fire policy/evacuation procedure when necessary.
2. To keep up to date with and attend training on revisions to information systems or changes in protocols for inputting of data in the light of local and national initiatives. To adhere to agreed protocols at all times.
3. To participate in individual performance review and personal development plans on an annual basis and to undertake training and development as identified within these discussions and as indicated by the requirements of the post.
Clinical Governance and Quality Assurance
1. To realise the importance of confidentiality when dealing with patients and staff, particularly when giving or receiving information over the telephone in accordance with the Data Protection Act.
2. Maintain the high standards of the service by contributing towards individual, team and service objectives and acting at all times in the best interest of patients.
3. To accurately collect, collate and input data to information systems as required by procedures to ensure that patient database records are up to date and accurate.
4. To participate in individual performance review and personal development plans on an annual basis and to undertake training and development as identified within these discussions and as indicated by the requirements of the post.
Information Governance
1. Post-holders will be required to comply with information governance related standards and policies at all times when dealing with confidential information, which includes any information relating to the business of the company and its service users and employees.
2. Post-holders are bound by a duty of confidentiality and must conduct their duties in line withe the NHS confidentiality code of practice, data protection act and freedom of information act.
3. Post-holders must maintain high standards of quality in corporate and clinical record keeping ensuring information is always recorded accurately and kept up to date. The post-holder must only access information, whether paper, electronic or in other media, which is authorised to them as part of their duties.
4. All information obtained or held during the post-holder's period of employment that relates tot he business of the company and its service users and employee will remain the property of the PCN. Information may be subject to disclosure under legislation at the company's discretion and in line with national rules on exemption.
5. Any breach of confidentiality or computer misuse could lead to disciplinary action, and in serious cases could result in dismissal. Breaches after the post-holder's employment has ended could result in the company taking legal action against them.
Health and Safety
1. To have responsibility for health, safety and welfare of self and others at work. This includes being conversant with trust health and safety policies and procedures and ensuring incidents, accidents and near misses are reported, taking part in the risk management process and carrying out tasks/using equipment only when competent to do so.
2. Be responsible for ensuring the general environment is clear of all hazards.
3. All staff have a responsibility to apprise themselves of how the prevention of the spread of infection relates to their role. They have a responsibility to infection prevention and control, and ensure that they comply with them in fulfilling their role.

This job description may be amended by management through consultation with the post holder in order to reflect changes in, or to the job.
Overview of your organisation
Wyre Forest Network of Independent Practices grew out of a mutual respect and vision of how primary care could work collaboratively without merger and was formalised in February 2018, when the group of practices signed a MoU to work in a mutually supportive manner. This pre-dated the Network Enhanced Service (by some 18 months) and meant that we had already developed a strong ethos of joint working, collaboration and trust. We have a PCN team which currently comprises of Clinical Pharmacists, Pharmacy Technincans, Care Home Nurses, ANPs, First Contact Physiotherapists and a Mental Health Practitioner.
