PCN Cancer Engagment Lead

Job summary

Due to funding from the Northern Cancer Alliance, we are looking for an enthusiastic PCN Cancer engagement Lead to join the Primary Care North Cumbria team. This role is hosted by Carlisle Healthcare on behalf of the 9 North Cumbria (PCN's) Primary Care Networks and is part of the newly created Primary Care North Cumbria Team.

Main duties of the job

You will work closely with the Primary Care North Cumbria Project Manager, Northern Cancer Alliance Cancer Delivery Lead, Clinical and Operational PCN/Practice teams to support service improvement and contribute to the development of North Cumbria PCN improvement plans.

Although the role will be home based, the requirements of this role will mean directly working with Primary Care colleagues and therefore the postholder will be required to work remotely across North Cumbrian Primary Care Networks and attend weekly face to face Primary Care North Cumbria team meetings.

About us

You will work closely with the Primary Care Network Operational Managers, Northern Cancer Alliance Cancer Delivery Lead, Clinical and Operational PCN/Practice teams to support service improvement and contribute to the development of North Cumbria PCN improvement plans.

Job responsibilities

To drive improvement in cancer outcomes, especially via earlier diagnosis, by building close working relationships with North Cumbria practices and PCNs To improve uptake of cancer screening, prevention and early diagnosis, by acting as the focal point for prevention and early diagnosis activities across North Cumbria.

Led by Carlisle Healthcare Manager, this role will support, educate and influence primary care teams, health professionals and local health systems to improve cancer services in line with the Integrated Care System and Northern Cancer Alliance workplan and the PCN cancer Direct Enhanced Service DES.

MAIN DUTIES AND RESPONSIBILITIES

The following is typical of the level of duties which the job holder could be expected to perform. It is not exhaustive and other duties may be expected to be performed by the post holder from time to time, where deemed necessary.

  • Build effective relationships with and actively engage Primary Care Networks/ Clusters and local cancer and public health stakeholders to raise awareness of the importance of cancer prevention, early diagnosis and screening and influence the content of local delivery plans.

Working closely with PCNs/Practices assist with the analysis and interpretation of statistics relevant to screening, prevention and early diagnosis

Work closely with (but not limited to) PRIMIS and PCN/Practice coding teams and GP Practice/PCN Cancer Leads to establish North Cumbria wide improvements in data collation and data review to highlight areas for improvement

Work closely with the Carlisle Healthcare Manager to encourage a collaborative approach across North Cumbria to ensure equitable service delivery across North Cumbria Place

Highlight (through access to PCN data) and act to increase uptake of the three cancer screening programmes in line with the PCN (DES).

To assist in the sharing of best practice, tools and resources and encourage PCNs to adopt quality improvement interventions that promote the prevention and earlier diagnosis agenda

Improve the use of FIT (Faecal Immunochemical Test) as a filter test in PCNs and increase the availability of a FIT result prior to a colorectal 2ww referral in line with the incentivisation of this practice in primary care

Facilitate the design and delivery of training to enhance knowledge of and interventions to improve cancer outcomes with a focus on prevention, early diagnosis, screening and addressing health inequalities.

Work in partnership with primary and secondary care partners to support and promote more effective ways of working across the patient pathway.

  • Support the delivery of local improvement projects in partnership with NHS partner organisations, in line with ICB/NCA workplan.
  • Keep up to date with policy, evidence and NHS/ public health cancer strategies, supporting the dissemination to local partner and primary care organisations in the area, as appropriate.
  • Work flexibly and collaboratively through the NCA region with other PCN Cancer Lead roles to support delivery including assisting in engagements and developing resources, as required.

Representation at PCN Engagement Staff Community of Practice Events

  • Provide monthly progress reports against agreed operational plan and identified performance measures to the PCN Operational Leads ICB and quarterly reports for Northern Cancer Alliance, in addition to other data, intelligence and observations, as required.
  • Provide at least 2 case studies per quarter to demonstrate the impact of either practice or PCN QI work.
  • Ensure accurate data records are maintained through timely input into the relevant data systems.
