Job summary
We are a GP surgery providing both primary and secondary care services. This role is working within the primary care service.
A position has arisen for an enthusiastic and motivated individual to undertake the role of Advanced Nurse Practitioner/Advanced Clinical Practitioner.
ANP applicants should be NMC Registered with Independent Prescribing. They should have a recognised Nurse Practitioner qualification working at degree level and be knowledgeable to lead in QOF, WPPC and Care Home work for example.
ACP applicants must hold relevant qualifications with their own professional bodies.
The role is full time but there are options for working these hours over 4 days.
There is also the possibility of management opportunities.
Main duties of the job
They should have experience in management of minor illness, minor ailments and injuries and chronic disease management.
They should be able to participate in patient telephone and /or video consultations to manage patient demand and advise patients in the safest and most appropriate way relevant to their problems.
They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

We are an enthusiastic friendly practice with approximately 15,000 patients and use SystmOne clinical system.
The premises are modern, purpose built, well-equipped with a friendly working environment.
You would be working with 2 GP partners, 4 salaried GP's, 2 Advanced Nurse Practitioners, a Minor Illness Nurse and a team of Practice Nurses and Healthcare Support Workers.
The clinical team is strongly supported by a large reception and administration team, line managers and senior managers.

2 Proactively identify, diagnose and
manage treatment plans for patients at risk of developing a long-term condition
(as appropriate).
3 Diagnose and manage both acute and
chronic conditions, integrating both drug and non-drug based treatment methods
into a management plan.
4 Prescribe and review medication for
therapeutic effectiveness, appropriate to patient needs and in accordance with

9 Take an active part in QOF management.
10 Prioritise health problems and
intervene appropriately to asses the patient in complex, urgent or emergency
situations, including initiation of effective emergency care.
11 Support patients to adopt health
promotion strategies that promote healthy lifestyles and apply principles of
self care. To give advice on Health
Promotion and Health Education.
12 Advise, support and administer
vaccinations for patients travelling abroad, where appropriate.
13 Support and manage health needs of
women presenting for family planning, cervical cytology or sexual health
consultation.
14 Triage patients (including Telephone
triage) to undertake practitioner led clinics e.g. CHD, Warfarin/Anti-Coagulation,
Diabetes, Rheumatology, Asthma and others as directed.
15 To undertake Audits, Appraisals,
Health Promotions and Teaching/Mentoring Staff.
16 Implement and participate in
vaccination programme as appropriate.
