Job summary
We are looking for an experienced General Practice Nurse to
join our PCN team, in an exciting new role to care for our population of
housebound patients across our network.
Main duties of the job
The nurse role will provide:
A year round full time nurse provision for our
housebound patients
Clinical focus will vary through the year to
meet operational needs, but will focus on these three main areas:
Providing a vaccination service for housebound
patients mainly Covid vaccination in spring and Covid and influenza in the
autumn, plus others as required including Shingles and RSV
Providing Proactive Care for eligible patients
this is a specific programme in Rotherham to provide a care plan review for
eligible patients, with follow on MDT for patients requiring additional
support. This involves a remote or face to face assessment, followed by
production of a written care plan
Providing annual reviews for chronic disease
management, QOF and medication reviews
Any other care required for housebound network
patients as directed
