Job summary
This role is working within Rotherham Central North Primary Care Network. This network includes Broom Lane Medical Centre, Greasbrough Medical Centre, Greenside Medical Centre,Kimberworth Park Medical Centre, Woodstock Bower Medical Centre. We are looking to expand our current SPLW team. We are looking for one permanent position and one part time fixed term contract to cover for a current project phase, we need the sucessful applicants of offer flexibility whilst we support the project.
Main duties of the job
Assist patients identified by GP Practices to access support from local VCS organisations and statutory services to meet their health and well-being needs.
Provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes. Co-produce a personalised support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
Main Duties and Responsibilities:
A Referrals - Promote Social Prescribing, build relationships, encourage self referrals
B Provide personalised support - meeting people on a one to one basis, Person centre approach, source of information,
C Support community groups and VCS organisations to receive referrals - forge strong links, support local groups
D Work collectively with all local partners to ensure community groups are strong and sustainable - work closely with GP practices, encourage patients to engage.
