Job summary

The Nurse Practitioner in primary care offers direct access to clients seeking health care. To be able to undertake initial assessments of any health problems likely to be encountered in a primary health care setting and, following assessment, can initiate treatments falling within her range of knowledge and skills. The Nurse Practitioner can admit people to the primary health care system, both by offering initial treatment herself and by referral to others, including physicians.

The Nurse Practitioner evaluates the effectiveness of treatment she has carried out at suitable intervals and, when appropriate, will discharge her clients from the primary health care system.

As a practitioner of nursing, she will offer an educational and supporting setting for clients to explore their health problems and will work with them for as long as necessary, enabling them to regain health.

Main duties of the job

  • Make professional autonomous decisions. Will have independent case loads.
  • Work closely with other members of the primary health care services.
  • Exercise the right to refer patients to appropriate services and resources
  • Maintain clinical notes in the patients medical records according to legal requirements.
  • Demonstrate knowledge and skill in interviewing and history taking and record appropriate details
  • Perform physical examinations
  • Select, carry out and interpret results of specific tests.
  • Develop with the patients an on-going plan for health,
  • Provide counselling and health education.
  • Co-ordinate the care provided for her patients by other carers.
  • Evaluate the effectiveness of the care provided in terms of expected outcomes.

  • Assist in the planning, development, implementation and evaluation of preventative programmes

Job description

Job responsibilities

  • Make professional autonomous decisions, for which she has sole responsibility.

  • Will have independent case loads, seeing patients with undifferentiated and undiagnosed problems, diagnose and, if necessary, seek advice and guidance from one of the medical practitioners whilst providing care herself.

  • Work closely with other members of the primary health care services, respecting professional role boundaries and acting in the best interests of patients, and liaise independently with agencies outside the practice.

  • Exercise the right to refer patients to appropriate services and resources within and outside the practice. These include other professionals by referral and an agreed range of services.

  • Maintain clinical notes in the patients medical records according to legal requirements.

  • Demonstrate knowledge and skill in interviewing and history taking and record appropriate details of her findings in the patients medical records.

  • Perform physical examinations, where necessary, and assessments, where appropriate, to recognize abnormalities and screen patients for risk factors and early signs of illness.

  • With in-house training, implement and evaluate treatment/intervention plans for each patient, based upon a comprehensive nursing assessment and seek and refer patients for treatment to medical practitioners in the practice, where necessary. For specific diseases, where necessary, organize recognized training, e.g. diabetes, asthma, epilepsy, hypertension etc.

  • Select, carry out and interpret results of specific tests.

  • Develop with the patients an on-going plan for health, with emphasis on preventative measures.

  • Provide counselling and health education as appropriate to facilitate health promotion.

  • With the patients` consent, discuss with other health care professionals and, if appropriate, relatives, the assessment/goals and treatment for patients.

  • Co-ordinate the care provided for her patients by other carers.

  • Evaluate the effectiveness of the care provided in terms of expected outcomes.

  • Collaborate with other members of the primary health care team in establishing protocols for the management and care of patients within all nursing and nurse practitioner`s fields of practice.

  • Assist in the planning, development, implementation and evaluation of preventative programmes to meet the primary health care needs of the practice.

  • Act as a nurse consultant to other members of the health care team, providing clinical advice and support to other professionals within the health care setting and to external agencies regarding social issues etc.
