Job summary
We are looking for an experienced pharmacist to develop our
Medication Review Service for Care Homes residents and those with severe
frailty within the Eston Primary Care Network (PCN).
With the support of the PCN Medicines Team and the wider
MDT, the successful applicant will work across our 4 GP practices supporting
patients to get the best out of their medication and reduce polypharmacy. Applicants must have excellent communication
skills and clinical knowledge and be ready to build relationships with key
stakeholders.
Primary Care experience is NOT essential. Successful candidates will be supported to
complete the CPPE Primary Care Education Support Pathway and the Independent
Prescribing qualification, if applicable.
Main duties of the job
Work
as part of a multi-disciplinary team in a patient facing role to clinically
assess and treat patients using expert knowledge of medicines
Be
a prescriber, or training to become a prescriber, and work with and alongside
the multi-disciplinary team across a Primary Care Network, take responsibility
for the care management of frail and care home patients and undertake
structured medication reviews to proactively manage people with complex
polypharmacy and high risk medications.
Work with patients and their carers to optimise medicines. Attend care home ward rounds and
run structured medication review clinics evaluating individual patients with complex
medication needs, agree treatment changes, seeing patients in clinic
appointments / home visits and discussing treatment changes with patients,
carers and community pharmacists.
To reconcile medicines at interfaces of care, most commonly discharge
from hospital. To include, identifying and rectifying any unexplained changes,
completing clinical medication review and arranging follow up such as follow up
tests, dose titration and medication supply where appropriate.
Lead on reducing medicines related hospital admissions and readmissions
by proactively targeting structured medication reviews.
Actively work with ICB to optimise quality of prescribing
Conduct
clinical audit and Quality Improvement as part of the multidisciplinary team.
Develop
relationships and work closely with other pharmacists

We are a
friendly and supportive well-established practice which has recently merged
with Manor House Surgery in Eston.
Normanby Medical Centre is located in Low Grange Health Village,
Middlesbrough. The practice operates
from a purpose-built premise. Training
practice for medical students from Newcastle and Sunderland and approved GP registrar
trainer from August 2021. Highly recommended as teaching practice by Medical
and Nursing students. Patient population is 22,700. Computer system is EMIS Web. (changing to
Systmone -February 2024) CQC rating
'Good'. High QOF achievement.
Our team
comprises of; - 12 GP Partners, 6 Nurse Practitioners, 10 Practice Nurses, 4
Health care Assistant, 2 Pharmacist, 1 Clinical Pharmacist (PCN) and a highly
skilled and supportive administration and reception team with good staff
retention.
We are the
lead employer for Eston PCN with a Partner as the PCN Clinical Director. We are supported by; - social prescribers,
pharmacists, Dietician, Physiotherapist, Physician Associates, Care coordinator, a Mental Health
Practitioner, Podiatrist, Health and Well-being Coach, Pharmacy Technician,
Admiral Nurse. We have strong working
relationships with our practice attached community services, CCG and local
practices. We provide our own enhanced
access on an evening and Saturday within our PCN. We are also part of ELM
Federation.

- Specialist knowledge (gained / working towards) through post graduate diploma in clinical pharmacy / therapeutics/ Primary Care Pharmacy Education Pathway or equivalent.
- Evidence of an on-going commitment to continuing professional development to maintain advanced theoretical and practical therapeutic knowledge
Desirable
- Registered independent prescriber

- Minimum of 3 years post registration experience in community primary, or secondary care pharmacy
- Evidence of continuing professional development in clinical areas.
- Developed consultation skills
