Job summary

We are seeking an experienced Advanced Nurse Practitioner or Paramedic, who will act within their professional boundaries, and provide comprehensive assessment to adults and child patients presenting with undifferentiated and undiagnosed minor illnesses, You will demonstrate safe clinical decision making and expert care, including assessment and diagnostic skills for patients within the Practice. The post holder will demonstrate critical thinking within the decision making process. You will work collaboratively with the wider multi disciplinary practice team to meet the needs of patients and supporting the delivery of policies and procedures.

You will join our friendly Advanced Practitioner Team consisting of two Advanced Nurse Practitioners and three Paramedics.

Main duties of the job

To provide the highest standards of care for patients and to provide assistance to the General Practitioners, Partners and wider Practice Team.

CLINICAL PRACTICE

  • Assess, diagnose, plan, implement and evaluate treatment / interventions and care for patients presenting with undiagnosed differential diagnosis.
  • Clinically examine and assess patient needs from a physiological and psychological perspective and plan care accordingly.
  • Proactively identify , diagnose and manage treatment plans for patients at risk of developing a long term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.
  • Work with patients in order to support both compliance and concordance to prescribed treatments.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimes, side effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Support patients to adopt health promotion strategies that promote healthy life styles and apply principles of self-care

Anticipate barriers to communication and take action to remove or reduce those barriers.

Maintain effective communication within the practice environment and with external stakeholders

Ensure awareness of sources of support and guidance (eg PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.
