Job summary

An exciting opportunity has arisen for an experienced Practice Nurse to join our large, multi-disciplinary team at Hall Green Health. Reporting to the Nurse Manager you will work alongside nursing team colleagues to achieve high standards of patient care, education and safety.

The successful candidate will be experienced in the provision of a wide range of Practice Nursing duties and ideally proficient in the delivery of care for long term condition management. You will be hardworking with a friendly and positive attitude and will take pride in delivering an effective and efficient service to our patients.

We are a friendly and supportive Practice with a strong focus on teamwork, continuous improvement and provide opportunities for ongoing training and development.

Main duties of the job

You will be responsible for the full range of treatment room / nursing duties working within your professional boundaries. You will actively contribute to wider nursing team duties including infection control and cold chain monitoring, service development and quality assurance activities.

We are a large, innovative GMS Practice based in Hall Green, Birmingham with a registered list size of 32,000. We have a good reputation in our locality and with patients, and a strong focus on patient care.

Our extensive clinical team includes 9 GP Partners, 17 Salaried GPs, plus a range of allied health professionals including Nursing, PAs, Clinical Pharmacists and Physiotherapists. Our supporting non-clinical team provide a full range of reception, administration and management functions. As part of Community Care Hall Green PCN we are further supported by a range of clinical and non-clinical AARS staff.

We work collaboratively, continually looking for opportunities to improve our services and actively support the wellbeing of one another. Our services have a good CQC rating with outstanding well led features and we regularly achieve high QOF performance. Clinical Activities

Adult and child immunisations and vaccinations including travel.

Health screening and promotion and long-term condition management including chronic disease reviews (e.g. diabetes, asthma, COPD).

Support patients with ECGs, blood pressure monitoring, hypertension.

Womens health e.g. contraception, cervical cytology, menopause advice

Assess, plan, implement and evaluate treatment for patients presenting with acute and chronic wounds, including removal of clips and sutures.

Direct communication with patients on matters relating to their ongoing health needs and personalising their care.

Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care, including taking opportunities to promote health and wellbeing wherever they arise.

Ensure high quality assessments are made of patients healthcare needs undertaking appropriate onward referrals where required.

Ensure specimens, laboratory tests and results are administered in accordance with policy and procedures.

Assist clinicians in the delivery of clinics e.g. minor surgery.

Respond to and support medical emergencies including BLS, anaphylaxis.

Contribute to the delivery of our extended hours access by participating in evening and Saturday clinical rota.

Support colleague development, work experience and / or student learning activities as required.

Provide advice and guidance to colleagues within the boundaries of professional practice.

Liaise with internal and external colleagues and agencies to support the provision of holistic patient healthcare.
