Job summary
We have a great opportunity for a suitable candidate to join our large, innovative Practice as a Planned Care Team Manager. You will be responsible for leading a team of Planned Care Administrators and associated staff, ensuring all aspects of our patient recall system and associated patient record processes are monitored and delivered to a high standard in accordance with agreed procedures, protocols and time scales. These processes are critical to the achievement of our Quality and Outcomes Framework, Enhanced Services, and other Practice contractual obligations, and the post holder will be required to ensure targets are met in line with expectations.
You will be good communicator, possess strong planning skills, with experience of handling sensitive information and working with the public, ideally in a General Practice setting.
Main duties of the job
The successful candidate will:
Take responsibility for the day-to-day management of the Planned Care team.
Work with the Operations Manager and Lead GP to carry out agreed processes to ensure QOF and other contractual targets are met.
Maintain patient recall system in line with practice protocols.
Maintain patient information and read codes as appropriate.
Establish and conduct clinical searches, interrogate data as appropriate and make recommendations for implementing clinics.
Lead and participate in working groups to achieve practice targets e.g. annual flu vaccination programme.
Co-ordinate the distribution and completion of information - writing and sending letters, text messages, emails and Blinx forms.
Submit daily and monthly reports, complete audits as required.
Ensure patient registrations are completed accurately and within service levels, following up queries, obtaining additional information where required.
Create and update relevant policies and procedures.
Participate in Quality Assurance meetings to review priorities for the team, communicate them, and plan and action these accordingly.
Escalate any conflicts within the team to the Operations Manager and support their resolution.
Complete any other duties commensurate with the role and Practice requirements.

We are a large, innovative GMS Practice based in Hall Green Birmingham with an increasing patient list of 31,500 patients. We have a good reputation in our locality and with patients, and a strong focus on patient care.
Our extensive clinical team incorporates 9 GP Partners and 17 Salaried GPs, plus a full range of allied health professionals including Nursing, HCAs, ANPs, Paramedic, PAs, Clinical Pharmacists and Physiotherapists, supported by a large non-clinical team providing a full range of reception, administration and management functions.
