Job summary

An exciting opportunity has arisen for a dynamic and enthusiastic registered Enhanced Practice Nurse to join our Primary Care Team. We are looking for an individual with experience and a specialist interest in Diabetic Medicine.The Enhanced Practice Nursewill be responsible for a number of clinical areas such as Health Promotion, Chronic Disease Management, Diabetic Management, as well as supporting the Nursing Management team in the reviewing of clinical policy and procedure.

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice.We are looking for an individual with a passion for diabetes who is motivated, willing to develop their knowledge and drive-up standards of care locally.

Main duties of the job

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)

Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan

Prescribe, review and instigate medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.

Work with patients in order to support compliance and adherence to prescribed treatments.

  • Relevant HCP qualification (e.g. RGN/HCPC)
  • Masters in advanced practice, or working towards
  • Independent Prescriber
  • Specialist in Diabetic Medicine

Desirable

  • Experience of working in Primary Care

  • Demonstrate the application of evidence based practice.
  • Experience in leadership role and management change
