Job summary

We are looking for a dynamic experienced practice nurse to join our friendly nursing team.

Our current nursing team team consists of a chronic disease nurse, 3 practice nurses, 1 nursing associate & 6 healthcare asistants.

Main duties of the job

Candidates will ideally have experience in a primary care setting, however applications may be considered if the relevant skills can be demonstrated. The role will include, long term condition reviews, wound care management, childhood and adult immunisations, womens health and the care of patients who attend general practice. You should be able to demonstrate on your CV the ability to cope in a busy environment. You will need to be a qualified nurse with an up to date NMC registration and evidence of continual professional development.

About us

Our team cares for over 28,000 patients. We are a training practice for SpecialtyGP Registrars. We have an excellent team of administrative staff to offer support and assistance to both our clinicians and our patients. We have a fantastically friendly team ethic and encourage a team approach in everything we do. We are supported by the district nursing team and health visitors who are based within our building. Additionally, we are fortunate to have on-site walk in blood clinics, mental health practitioners, MSK practioners, Physician Associates, Nursing Associate and in-house pharmacists to assist in prescribing administration. We have a range of PCN services to support our population needs. We use Emis Web and Docman.

This is not a comprehensive list but outlines minimum requirements: To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF. To undertake cervical screening. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages. To co-ordinate the provision of travel medicine services. To request pathology services and process pathology results as required. To undertake wound management. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
