Job summary
We
are looking for a Chronic Disease Trained Nurse qualified to provide COPD,
Asthma, Diabetes, CHD. We would ideally
like a Nurse who is able to carry out travel assessments, family planning,
immunisations, and smears.
We
are flexible on the number of working hours and days for the right candidate.
The successful candidate will be joining a friendly team and be supported by 2
Chronis Disease Nurses, 2 Treatment Room Nurses and 2 Health Care Assistants.
Our
recall process is structured in a true medication and chronic disease review
to minimise the number of appointments for the patients benefit as well as
reducing demand.
The
successful candidate will be working in an environment where they will be
supported by a strong team with an open-door policy. We have weekly meetings
which are an opportunity to discuss complex patients and any positive or
adverse events which could impact on the care we provide.
Main duties of the job
a.
To
act as the Infection Prevention Control (IPC) lead and to work to the IPC
Policy providing audits, training, support and evidence of compliance to the
practice manager
b.
To
assess the needs of patients ensuring the provision of healthcare is
appropriate, incorporating evidence-based practice
c.
To
develop, implement and embed health promotion and wellbeing programmes
d.
To
implement and evaluate individual treatment plans for chronic disease patients
that promote health and wellbeing
e.
To
identify, manage and support patients at risk of developing long-term
conditions, preventing adverse effects to patients health
f.
As
required, to diagnose and manage acute and chronic conditions
g.
To
prescribe medication for patients in accordance with PGDs and own scope of
practice
h.
To
co-ordinate the provision of travel medicine services
i.
To
oversee the provision of chronic disease clinics, delivering patient care as
necessary, referring patients to secondary/specialist care as required
j.
To
maintain accurate clinical records in conjunction with extant legislation

To

About us
Devonport Health Centre is
located on the Cumberland Campus between the Peninsula Dental School and the
Cumberland Centre. We have a list of just under 8000 patients and historically
the area is deprived. We are one of the Deep End Practices supporting complex
patients who not only need medical support but also face social challenges.
Our
aim is to work together to help the local population to stay physically and
mentally well, to get better when they are ill, and to remain as independent as
they can until the end of their lives. To provide accessible healthcare to a
high standardlocally to our patients.
We
have diversified our team due to the GP shortage across the Southwest and offer
appointments and triage from the Minor Ailments Nurse undertaking the ACP
course, Clinical Pharmacist, 2 Mental Health Practitioners in addition to on
site and remote Locum GPs.
We
operate a triage model for acute on the day problems to direct our patients to
the right clinician, for the right problem, in the right amount of time,
allowing the right amount of time.
We
have external providers hold additional clinics at the Health Centre which
include Cardiology, Dermatology, Minor Operations, Vasectomy and Clinical
Genetics.
