Job summary

An exciting opportunity has arisen to join our multi-disciplinary PCN team. We have an established model of working with NQ GP's providing support, with additional Clinical supervision from established and experienced GP's, aiding a smooth transition in your first role working in Primary Care as a GP.

Working for Cheltenham Peripheral Primary Care Network (CP PCN), you will be working with three of our 5 GP practices who collectively care for 54,000 patients. Our GP practices are located around the peripheral of Cheltenham extending in the south from Leckhampton to Bishops Cleeve and Winchcombe.

The PCN works innovatively to provide a holistic approach to our patient population. The PCN team already includes; Social Prescribing Link Workers, Care Coordinators, Clinical Pharmacists, Pharmacy Technicians, GP Assistants and a Mental Health Nurse.

Main duties of the job

The ideal candidate should be a newly qualified GP within the first two years of gaining their certificate of completion of training (CCT), have full GMC registration and be included on the NHS England GP performers List.

To perform as an autonomous general practitioner working independently and with other health care professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting. The successful candidates will rotate between one and three PCN Practices providing you the opportunity to experience the varied dynamics of each practice. As a newly or recently qualified GP you will benefit from daily support from experienced GP colleagues.

The role involves both clinical and administrative work and we utilise the System One Clinical System across the PCN. Allocated administration time for each clinical session will be provided as per recognised guidance.

The post holder will be professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct ensuring the safety of patients and the quality of their care.

In recognition that this role is designed for newly qualified GP's we would offer the opportunity to receive regular formal mentoring from an experienced GP.

The post holder will need to be capable of independent travel across the Cheltenham Peripheral PCN and work flexibly to meet the needs of the Business.

About us

Cheltenham Peripheral PCN is made up of 5 GP practices. Leckhampton Surgery, Sixways Clinic, Cleevelands Medical Centre, Stoke Road Surgery and Winchcombe Medical Centre, all local to Cheltenham combining rural and urban practices. All of our 5 practices are training practices and are well placed to support a newly qualified GP.

Job responsibilities

To undertake a variety of duties including Surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.

Manage a broad range of acute and chronic illnesses, ensuring that care is delivered in line with evidence-based guide lines and best practices.

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.

Assessing the health care needs of patients with undifferentiated and undiagnosed problems.

Screening patients for disease risk factors and early signs of illness

Participate in the management of chronic diseases, such as diabetes, hypertension, asthma, and COPD, ensuring that patients receive optimal long-term care.

In Consultation with patients and in line with current practice disease management protocols, developing care plans for health.

Providing counselling and health education

Recording clear and contemporaneous consultation notes to agreed standards

Collecting data for audit purposes.

Compiling and issuing computer-generated acute and repeat prescriptions

Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.

The post-holder will be required to visit patients during surgery time in an emergency if clinically indicated.

The post-holder will undertake a share of home visits per week as allocated.
