Job summary
Kearsley Medical Centre are seeking to recruit a Paramedic Practitioner to work up to 30 hours per week over five days to join our clinical team, providing assessment, diagnosis and treatment of our patients at the practice, in patient's own homes and within our local care homes.
Main duties of the job
Key Responsibilities and Scope
To provide
paramedic practitioner activities in a primary care setting to support the GPs,
holding clinics and visiting patients within their own homes.
Under the
supervision of the GP, provide medical care, assessments, evaluation and treatment
for patients both in clinics and in their home.
To work with the Duty
Team as required, prioritising the needs of patients and making referrals as
necessary.
To make
differential diagnoses using decision-making and problem-solving skills,
working under the supervision of the GPs.
To order
necessary investigations and provide treatment and care both individually and
as part of the primary care team and through referral to other agencies.
To screen
patients for disease risk factors and early signs of illness
To develop with
the patient an on-going nursing care plan for health
To work
collaboratively with other health care professionals and disciplines
To actively
pursue policies that will promote healthy lifestyles for patients: promote health
education and participate in relevant sessions within the practice.
To continuously
improve personal specialised skills through audit, research, review and
fulfilling training needs.
Ensure the
clinical computer system is kept up to date with accurate details recorded and
amended
About us
Kearsley Medical Centre is based in South Bolton, close to the A666 and provide care to approximately 13,500 patients. We are a training practice and support trainee GPs as well as our employed staff in the development of their career.
We have a friendly team of 40+ members of staff, plus additional network staff, who work collaboratively to delivery best care to the patients in our community.
Our clinical team consists of 6 GP Partners, 2 Salaried GPs, 2 ACPs, 3 Practice Nurses, 1 Nursing Assistant Practitioner, 1 PCN Paramedic, MSK and Mental Health Practitioners, and a Pharmacy team.
Our clinicians are supported by reception, admin, clinical admin and management teams.

Reporting
This post holder reports and is accountable to the GP Partners for clinical activities and to the Practice Manager for organisational issues.
Key Activities and accountabilities
1. To offer care complementary to that offered by the GPs and other Practitioners in the Practice
2. To undertake visits to patients in their own homes and to nursing homes if required during urgent / acute episodes of care.
3. Participation in proactive care of nursing and residential care home patients including attendance at MDTs
4. To monitor and evaluate standards of care through appropriate audit, taking appropriate remedial action if standards are not achieved
5. To be accountable for care given within Practice policies, protocols, and procedures
6. To maintain accurate and comprehensive records with specific regards to confidentiality
7. To advise and support other members of the team whilst working as a flexible and conscientious team member
8. To attend appropriate clinical update training and general and specific paramedic skills and training in order to maintain knowledge and skills
9. To be aware of national and local targets; initiatives; in relation to own practice and to apply these in the clinical setting
10. To recognise professional boundaries and ensure these are maintained
11. Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
12. Contribute to the effectiveness of the team by reflecting on own and team activities

14. Undertake the collection of pathological specimens including intravenous blood samples, swabs etc
