Job summary

An exciting opportunity has arisen for a Pharmacy Technician at Ivel Valley South PCN, supporting delivery of the new Network Contract DES Service specifications and IIF targets.

Ivel Valley South cover the following surgeries:

Dr Baxter & Partners, Larksfield and Arlesey Branch Surgery and Lower Stondon Surgery

Main duties of the job

The post holder will be joining a team of 5 clinical pharmacists and 1 technician taking responsibility within the network supporting areas of chronic disease management, assisting with prescription and medication queries and undertaking clinical medication reviews to proactively manage patients and directly tackle over-medication of patients, reduce waste and promote self-care.

Job description

Job responsibilities

-Implement and support electronic prescribing and repeat dispensing in appropriate patients including working with the community pharmacy teams to identify appropriate patients.

-To support the training and development of the Prescription Clerks and to take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.

-Support Care homes/residential homes as a point of contact for weekly support to -include: prescription queries, synchronisation, medicines wastage.

To undertake medicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly.

-Support the prescription clerks by helping with queries relating to prescription requests and look for ways to improve the efficiency and safety of the prescription issuing process, Complying with legal and professional requirements and with safe systems of work for re-authorising repeat prescriptions.

-Interface with community and hospital pharmacy colleagues on medicines related issues.

-Implement and support use of monitored dosage systems and support the district nursing team with medicine optimisation for housebound patients and liaising with community pharmacy staff.

-Carry out medication reviews whilst in patient records e.g. dose optimisation and medicines synchronisation. Bring queries to clinical team to include under or over usage.

-Review out-patient letters which include changes to drug treatment and make changes to patients repeat medication list as appropriate (or query prescribing if not in accordance with Formulary). Liaising with patients and community pharmacy regarding changes.

-Working with the PCN Clinical Pharmacist and Prescribing Lead GP to contribute to the review and development of repeat prescribing protocols / Standard Operating Procedures to improve the efficiency and effectiveness of repeat prescribing systems.

Person Specification

Qualifications

Essential

  • Professional registration with GPhC.
  • BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
  • Evidence of continued professional development (CPD)
  • Experience of working as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy.

Desirable

  • Relevant advanced theoretical and practical knowledge of general practice and evidence based medicine
  • An appreciation of the NHS 5 year plan
  • An understanding of the structure of Primary Care Networks
  • Awareness of systems to support management of patients in a primary care setting, delivering pharmaceutical input and support in the context of pathways of care and the business of the organisation
  • Good clinical pharmacy knowledge including medical terminology
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing
  • Computer literate with an ability to use the required GP clinical systems and Microsoft office packages
  • Able to analyse and interpret prescribing data
  • Has attention to detail, able to work accurately, identifying errors quickly and easily
  • Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines
  • Excellent verbal and written communication skills with the understanding of data protection
  • Works effectively independently and as a member of a team
  • Flexible approach to meet service needs and ensure a stakeholder focused response
