Specialist Practice Nurse Ambulatory Care

Job summary

We are looking for an experienced nurse to join our PCN in an exciting new role that has been developed to work alongside our multi disciplinary team, and also work closely with the PCN's Practices and their teams to provide care and treatment to patients presenting in primary care and community care settings, including their own homes and care homes.

Main duties of the job

The postholder will work autonomously and within their professional boundary to deliver planned ambulatory care services to the PCN's patient population, focussing on patients who are hard to engage, taking an holistic view and adressing a wide variety of needs.

The postholder will provide expert advice to patients, carers and colleagues to ensure the maintenance of clinical excellence and provide advice in respect of ongoing care needs, being accountable for their own professional actions within professional guidelines.

The postholder will be supported by the PCN's Clinical Director and work collaboratively within the multi disciplinary team to meet the needs of patients, support the delivery of policy and procedures and improve our service delivery.

Job description

Job responsibilities

Undertake clinical practice at an advanced level using knowledge, within scope of competence, to deliver holistic care to people accessing primary healthcare services, largely in their own homes/care homes

Work with the PCNs practices to identify and manage hard to engage patients

Assess, diagnose, plan, implement and evaluate treatment/interventions for patients with long term conditions

Refer patients directly to other services/agencies as appropriate

Provide assessment, screening treatment services and health education advice

Undertake pathological specimens and investigatory procedures

Assist in and perform routine tasks related to patient care

Work with other members of the practice teams and wider PCN, to plan and deliver ambulatory care services, promoting integrated and seamless pathways of care

Support and mentor other colleagues within the PCN who are seeing similar patients.

Provide safe, evidence-based, cost-effective, individualised patient care

Promote health and well-being

Contribute to the PCNs practices achieving quality targets to sustain the high standards of patient care and service delivery

Develop and establish new patient services and participate in initiatives to improve existing patient services

Qualifications

Essential

  • Have appropriate registration with the NMC
  • Current UK Driving Licence

Desirable

  • Independent Prescriber

Experience

  • Ability to develop care/management plans for patients
  • Knowledge of and experience of working with patients with long term conditions
  • Able to request and interpret investigations and results
  • Able to work and make decisions autonomously, working within own professional boundaries
  • Clinical leadership skills
  • Minimum of 3 years post registration experience
  • Evidence of CPD
  • Able to evaluate the safety and effectiveness of own practise

Desirable

  • Understanding of NHS strategy, particularly related to primary care
  • Knowledge of primary and community healthcare
  • Clinical audit knowledge and skills
  • Knowledge and experience of safeguarding
  • Experience of working with SystmOne
