Job summary

To develop, co-ordinate and provide a highly specialist diabetes care service, while working in partnership with patients and their carers/relatives and clinicians at the practice. To be a clinical and educational resource and provider of specialist care for patients and their health professionals across the healthcare community.

Job description

Job responsibilities

Provide specialist diabetes care to patients within the primary care setting, in line with NICE guidelines and local protocols.

Conduct comprehensive assessments and reviews of patients with complex diabetes, including insulin management and medication titration.

Support GPs and practice nurses with treatment plans and clinical decision-making.

Lead and contribute to the delivery of structured diabetes education programmes (e.g., DESMOND).

Promote self-care and lifestyle management among patients to support glycaemic control and reduce complications.

Work closely with the wider multidisciplinary team including dietitians, podiatrists, and secondary care DSNs.

Undertake audits, quality improvement initiatives, and contribute to QOF/PCN targets related to diabetes.

Provide training and clinical supervision to practice staff in diabetes care and management.

Keep accurate, contemporaneous records on clinical systems (e.g., SystmOne)

Engage in patient recall and proactive case-finding to identify those at risk of poor control or complications.

Essential

  • Qualifications / Training
  • Registered Nurse (NMC)
  • Post-registration qualification in diabetes care or willingness to undertake
  • Evidence of ongoing CPD in diabetes or long-term condition management
  • Experience
  • Minimum 2 years post-registration experience
  • Experience managing diabetes in a primary care or community setting essential
  • Experience with insulin initiation and titration
  • Experience delivering structured education or group sessions
  • Skills and Knowledge
  • In-depth knowledge of diabetes management and national/local guidelines
  • Excellent communication and patient engagement skills
  • Competent in clinical examination and risk assessment
  • IT skills, including use of clinical systems (EMIS/SystmOne)
  • Ability to work independently and as part of a multidisciplinary team
