Role Summary
To provide a comprehensive nursing service to the practice population to meet healthcare needs, including specialist clinics.
To work as an independent practitioner providing evidence based nursing care appropriate to the patients needs.
Will be based at the surgery but will also be required to visit patients in their own homes on occasions.

Reviewing, assessing and interpreting risk factors relating to Chronic Disease management
Assessing and interpreting symptoms in Nurse Led Clinics
Communications and relationships
Regularly communicates with various people through face-to-face contact or via the telephone using verbal and non verbal methods. Will need to communicate with patients whose first language is not English
Should recognise the importance of effective communication within the team.

To provide assessment, screening and treatment services which are relevant to the patient.
Develop and organise the chronic disease management programmes for which you have had specific training, which could include:
Asthma
Diabetes
CHD
COPD
Hypertension
Cytology
To assess patients healthcare needs through telephone or face to face triage, offering appropriate treatment and advise.
See patients with minor ailments as agreed appropriate for nurse intervention, follow practice policies for the treatment of such ailments
To develop and carry out clinical audit and research in relation to patient care and nursing practice, which will help to attain and demonstrate a high standard of clinical practice.
To develop protocols and guidelines with Gps for use in the practice
As a training practice you will be required to have medical students and Gp registrars sitting in with you as part of their training and development.
To participate in the administrative and professional responsibilities of the practice team and to assist in the formulation of practice philosophy, strategy and policy.
Responsible for ensuring the nurses medical equipment maintenance programme is up to date and that all equipment is safe and that stocks are maintained
Responsible for maintaining own professional development by attending courses and study days as arranged with the partners
