Job summary

Wansford and Kings Cliffe Practice has a full time vacancy for an experienced Practice Nurse to lead and grow our existing nursing/HCA team.

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population across both Wansford Surgery and our branch surgery, Kings Cliffe.

As nursing team lead, the post holder is accountable for the nursing service delivery. They will lead and manage all the nursing resource, develop the nursing team and work closely with the practice management and GPs to deliver the practice priorities.

Clinically the focus of the role is the delivery of evidence based care for patients with long-term conditions along with offering management and preventative nursing interventions to all of our patients. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making. They will work collaboratively with the whole practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership and direction for the nursing team.

To ensure the highest standards of care are provided for patients.

To manage and develop a clinical caseload, dealing with patients needs and in accordance with evidence based practice and national and practice protocols.

To deliver a high standard of patient care using specialist practice nursing skills.

Lead Nurse Role

Demonstrate kind and compassionate leadership within the nursing team and the wider GP surgery

To provide clinical leadership within the nursing team.

Act as a role model

Support members of the nursing team to undertake mandatory and statutory training requirements

Provide clinical and educational advice to support the development of the team and the range of services provided

Facilitate/conduct nursing team appraisals

Responsible for nursing team meetings

Wansford Surgery is a thriving and well established independent Practice serving circa 10000 patients across 120 square miles of rural territory nestling between Stamford and Oundle. We are a highly regarded training practice, with a strong ethos of education and professional development to enable us to deliver the highest possible standard of care.

Our current nursing team comprises 1 full time and two part time practice nurses, a full time HCA, a part time phlebotomist, a full time Diabetes Specialist Nurse and a full time Cardiology Specialist Nurse.

Job description

Job responsibilities

The following are the core responsibilities of the Lead Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

a. To assist the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the practice manager

b. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice

c. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

d. To develop, implement and embed health promotion and wellbeing programmes

e. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

f. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

g. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

h. As required, to diagnose and manage acute and chronic conditions

i. To provide guidance to patients on the use of prescribed and over-the-counter medications regarding side effects and dosages

j. To co-ordinate the provision of travel medicine services

k. To request pathology services and process pathology results as required

l. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

m. To maintain accurate clinical records in conjunction with extant legislation

n. To ensure read codes are used effectively by all members of the nursing team

o. To ensure the appropriate maintenance and use of chronic disease registers

p. To ensure compliance with the practice chaperone policy
