Job summary
Westway Surgery is a friendly, thriving,
and innovative practice with an urban population, conveniently located near the M4 and
close to Cardiff City Centre.
We are currently seeking an exceptional pharmacist to
enhance our patient-centred care approach, conduct medication reviews with
meaningful interventions, contribute to research and deliver patient care
within scope of practice.
We offer a wide range of clinical services including minor surgery, contraception, nursing home DES, Warfarin, NOAC, insulin initiation, transgender service and substance misuse shared care.
In October we will
merge with Lansdowne Surgery to cover 16,000 patients and further expand our
multidisciplinary team and opportunities for development. GP training will
complement our current hosting of foundation pharmacists and general practice
nurses in training.

Main duties of the job
The
following are the core responsibilities of the clinical pharmacist.
Practice as an independent prescriber or working towards the status and working within
scope of practice
Carry out face-to-face and telephone consultations with
patients with minor illnesses
To provide medication review services with patients in
the practice and during domiciliary visits to the local nursing home
To provide advice and support for patients with
self-limiting conditions
Encourage cost-effective prescribing within the practice
Review all safety alerts ensuring the required actions
are taken
Review medications for newly registered patients
Implement and embed a robust repeat prescribing system
To provide subject matter expertise on medication
monitoring, implementing and embedding a system
Carry out targeted medication reviews, reauthorise meds, for high-risk
patients
Review and implement safe prescribing systems for
vulnerable patients, improving compliance
Partake
in audit as directed by the audit lead
Provide
education on medicines for the clinical team
Review
the latest guidance ensuring the practice conforms to NICE guidance and local pathways
Act
as the practice point of contact for all medicines related matters,
establishing positive working relationships

addition, our Cluster has established weekly multi agency multi - disciplinary
team meeting and an active Hub delivering welfare checks and health
monitoring.
