Job summary
As a Social Prescriber link-worker in Torbay, you will join a group of dynamic primary care networks, helping to make a real difference to peoples lives by empowering them to take control of their health and wellbeing, using a holistic approach and connecting people to community groups and statutory services for practical and emotional support.
This is position is 30-37.5 hours per week and salary is £22,486 -£26, 886 per annum WTE depending on experience. Main duties of the job
- Promoting social prescribing, its role in self-management, and the wider determinants of health.
- Meet people on a one-to-one basis, making home visits where appropriate within organisations policies and procedures. Give people time to tell their stories and focus on what matters to me. Build trust with the person, providing non- judgemental support, respecting diversity and lifestyle choices. Work from a strength-based approach focusing on a persons assets.
- Forge strong links with local VCSE organisations, community and neighbourhood level groups, utilising their networks and building on whats already available to create a map or menu of community groups and assets. Use these opportunities to promote micro-commissioning or small grants if available.

About us
You will be based in GP practices across our Baywide PCN (Compass, Pembroke, Chilcote), working across different practices as well as completing home visits as necessary. You will be embedded as a member of the practice teams, receiving full support and training in your role from the existing Social Prescriber link workers and local training providers. A qualification and or/interest in mental health is desirable.
