Practice Nurse / Nursing Associate

Do you want to be part of an innovative and collaborative nursing team? We have an exciting opportunity for a Practice Nurse or an NA to join our team. As a member of our team, you will be fully supported in offering high quality, patient focused services.

Based at Adelaide Street Surgery (but also possibly working across our 2 sites), in the most deprived area of the city brings it challenges but also its rewards - you can make a huge difference. The hours of this role are between 20 - 24hours per week. There is requirement to cover in times of annual leave and sickness within the team. We offer a competitive salary - commensurate with experience.

A love of cake and good humour also useful!

Main duties of the job

The successful applicant will be able to (with training) provide all aspects of Practice Nurse or NA role including dressings, cytology, immunisations, chronic disease management and long-term conditions - Working alongside a wider nursing team of Phlebotomy, HCAs, Extended skills HCAs, NAs, Practice Nurses and Advanced Clinical Practitioners. An understanding and empathy with the challenges that substance misuse, mental health and addiction and working within a deprived population brings are important to us as a team.

Main Responsibilities - General

All aspects of wound management from simple to complex

Assist with minor surgery

Support Chronic Disease screening including CVS/diabetes/asthma/COPD

Management of above in accordance with Practice Policies and PGDs / PSDs

Health promotion general and links with chronic disease management. Implement and develop health promotion advice in accordance with current guidelines

Refer as appropriate to other members of the team and other agencies

Promote a positive and pro-active image of the practice and the Primary Health Care Team

Maintain stocks of medicines, drugs, dressings and equipment in treatment room and re-order when necessary. Keep up to date on changes and advise where appropriate.

Initiate, apply and advise on infection control according to local and national guidelines including ensuring all relevant equipment is appropriately sterilised and stored.

Administer under PGD / PSD all prescription only medicines. Ensure safe and appropriate administration, storages be able to identify and deal with adverse reactions including anaphylaxis. Attend regular Life Support updates.

To provide an optimum level of care for all patients and highlight any causes for concern or areas where this may be compromised to the appropriate practice person

Liaise with relevant GP or other healthcare professional or agency to ensure best patient care and maintain two-way communication

Have good basic awareness and understanding of mental health, recognise the psychological needs of patients presenting with depression, suicidal tendency etc

Understand and be aware of impact of substance misuse, domestic violence/abuse, addictive behaviour, vulnerable adults be able to refer to other agencies in consultation with GP.

Work effectively, participate in and contribute positively to the nursing and the wider Primary Health Care Team ensuring good communication about all aspects of the practice

Monitor and maintain consistency of professional standards of yourself and the PHCT and report concerns appropriately

Recognise, respect and support the development needs of others in the PHCT

Teach and supervise others including unqualified staff

Initiate and carry out audit.

Clinical Practice Development

Participate in and contribute to evidence-based practice and initiate and participate in clinical supervision.

General Staff Duties

All members of the Primary Health Care Team are required to attend and contribute to meetings / away days and ongoing training requirements.

Be responsible for identifying need and updating qualifications according to current professional guidelines. Keep abreast of current trends and developments in professional matters.

The post holder is required to conform to all Adelaide & St Levan Surgery policies and to attend any training sessions as necessary.
