Job summary

Due to relocation we have an exciting opportunity to join our team in inner city Plymouth.

We are looking for a motivated, enthusiastic ANP who is committed to delivering excellent primary care services to disadvantaged populations across our 2 sites

You will be well supported by our clinical team of experienced GPs & ACPs, Pharmacists, Paramedic, Physiotherapists, Social Prescribers, PCMHW & excellent Nursing & Admin teams. All with a love of cake, a good night out & staff room banter.

We work closely with the Outreach and HIPP service, providing clinics for homeless population & complex lives at various Hostels in Plymouth along with Drug & Alcohol Services. There is opportunity to develop skills in mental health & addiction medicine, ayslum & refugee population further training is provided as required.

A training practice, we deliver both medical student & GP registrar teaching, nursing placements & non clinical apprenticeships.

The work is interesting & rewarding. This is an opportunity to develop specialist skills & career development. There is a max of 8 sessions - over 4 days with option of a part time role to suit the needs of the practice.

Main duties of the job

The post holder would be an experienced practitioner with independent prescribing qualification. Using clinical knowledge and skills you will have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients, both on the phone, in person or in the home.

The surgery is part of Waterside PCN, a flourishing and successful and to that end offer Extended Hours which the post holder would be required to be part of that rota.

A two site, 7 partner practice, covering a total population of 12,500 patients. You would be joining a full multi- disciplinary, fully staffed team. We use Systm One and Klinik and Accurx to support our appointment triage system.

Key clinical responsibilities:

Triage, assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Proactively identify, diagnose and manage both acute, chronic and long term conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Communication:

Utilise and demonstrate appropriate communication styles, to ensure patients are fully informed and consent to treatment

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

Maintain effective communication within the practice environment and with external stakeholders

Act as an advocate for patients and colleagues

Ensure awareness of sources of support and guidance (eg PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate
