Vaccination and Immunisation Care Coordinator

Job summary

The post is part of the Affinity Care Primary Care Network Team, and will provide day to day support in the bookings of appointments for patients linked to the roll out of our vaccinations (vaccs) and immunisation (imms) programmes, including the planned COVID Vaccination programme, the influenza programme, Shingles, baby vaccs and imms, other adult immunisation programmes such as HPV, Pertussis in pregnancy, Meningitis ACWY and any catch-up programmes as introduced from time to time.

Main duties of the job

Handle enquiries from the general public around scheduled vaccination programmes including but not limited seasonal influenza, Covid vaccinations and child immunisations.

Proactively follow up with patients to book in their vaccinations in line with eligibility criteria as defined by the NHS.

Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.

Job description

Job responsibilities

Understand the eligibility and requirements for all vaccines, immunisations and screening.

Support the development of searches within SystmOne to clearly identify eligible cohorts and provide accurate data relating to the various programmes

Use SystmOne searches to identify patients eligible for the vaccines, immunisations and screening to support workload planning and booking activity

Co-ordinate the booking of appointments for these patients into appropriate clinics within agreed timescales

Co-ordinate the planning and booking of care home and housebound patients in partnership with the Complex Care Team (CCT)

Support the PSM and Nurse Lead to ensure all vaccines are ordered pro-actively to appropriate levels and stock returns submitted using agreed systems

Support the PSM and Nurse Lead to ensure the distribution of vaccines across all sites as relevant to planned activity

Work with the PSMs and Care-Cordinators to provide rotas for both the booking team and vaccination / immunisation / screening teams

Support the administrative and vaccination teams to provide a quality system of work that is effective in its use of team resources

Apply proven approaches to encourage attendance and reduce DNAs

Constantly seek service improvement, mindful of the patient experience and patient safety balanced with efficient use of our resources

Maximise the use of new technology and methods of communication with patients

See things from a patient perspective; identifying ways to improve our systems to enhance the patient experience

Contact eligible patients, provide supporting information as necessary to patients and book appointments as appropriate

Capture such knowledge in a way that it can be shared with other care coordinators and inform other teams within Affinity Care

Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.

Escalate all relevant issues and information to their Line Manager, ensuring they are aware of any developments impacting the operational functioning of the service

Support the collation of all activity data and reporting information as required

  • GCSE English Language, Grade C or above (or equivalent)

  • NVQ Level 3 or equivalent level of knowledge in office procedures and systems or customer service
