Job summary
We are seeking an experienced and motivated Community Nurse to join our community home visiting team. This is an exciting
opportunity to work in a dynamic team of GPs, doctors, nurses and healthcare
assistants delivering high-quality, person-centered care in patients own
homes.
The successful candidate will provide advanced clinical expertise, leadership,
and service development to ensure patients receive safe, effective, and
compassionate care that supports independence, prevents unnecessary hospital
admissions, and enables end-of-life care at home where appropriate.
The role is Monday to Friday 37.5 hours per week
Main duties of the job
- Deliver advanced nursing assessments,
care planning, and treatment to patients in their homes, managing complex and
high-risk caseloads.
- Ensure effective caseload allocation,
safe staffing, and prioritisation of urgent visits.
- Work collaboratively with GPs, palliative
care services, therapists, and social care colleagues to provide integrated,
holistic care.
- Ensure compliance with safeguarding,
infection prevention, NMC standards, and Trust governance frameworks.

- - Car driver with access to a vehicle.
