Dean Cross Surgery is located in Plymstock, Plymouth with 10,200 registered patients. The practice has a Good CQC inspection and has excellent patient feedback, with staff feeling valued and appreciated by our patient group.
The Practice team is led by 3 GP partners who are supported by salaried GPs, an established ANP team, skilled Nursing & HCA team and administration team. We have the following professionals working alongside us, Mental Health Nurse, Wellbeing Coach, Social Prescribers, Pharmacists, Advanced Physiotherapist, First Contact Physio and Nursing Home Team.
Main duties of the job
We are looking for an experienced registered nurse or allied health professional who will be responsible for acting within their professional boundaries providing care to presenting patients, from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
You will need experience in safe clinical decision-making and providing expert care, with knowledge of assessment and diagnoses of patients within general practice or similar setting via telephone triage, face to face appointments or home visits. The successful applicant will be required to prioritise and triage the needs of the patient, making any necessary referrals for investigations in the appropriate manner. Our ACP team visit patients in their own home as part of the home visit rota, confidence and/or experience of delivering care to patients at home is an important element of this role.
We expect you to work collaboratively with the general practice team to meet the needs of patient, supporting the delivery of policy and procedures, and providing nurse leadership as required.

- Clinically examine and assess patient needs from a physiological and psychological perspective based on advanced nursing knowledge and skills, and plan clinical care accordingly, make differential diagnosis using decision making and problem solving skills
- Make professional autonomous decisions within the boundaries of competence, acknowledging limitations
- Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
- Independently prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national, local and practice protocols, and within scope of practice
- Order necessary investigations and provide treatment and care both individually and as part of a team, and through referral to other agencies
