Job Summary
The Paramedic Practitioner will play an integral role within the multidisciplinary team at Tower House Surgery, working both autonomously and collaboratively to deliver high-quality care to our registered population. This will include face-to-face consultations, home visits, clinical assessments, diagnosis, treatment, and appropriate onward referrals.
You will support the Practice Manager and PCN Manager in delivering safe, evidence-based care, aligned with local and national policies. The post holder will also be involved in shaping clinical procedures and supporting the practices quality improvement and governance processes.
Main duties of the job
Core Responsibilities
Working within your scope of practice, you will:
Conduct clinical assessments, diagnosis, and management of patients presenting with a wide range of conditions.
Undertake home visits as required.
Lead in the management of minor illness and urgent care presentations.
Monitor and review long-term conditions where appropriate.
Refer patients to other healthcare professionals or services where clinically necessary.
Accurately record consultations and interventions using clinical coding and local systems (e.g. EMIS Web).
Work in accordance with local clinical pathways, patient group directives (PGDs), and prescribing guidance.
Provide health education and health promotion advice during opportunistic encounters.
Ensure continuity of care by arranging appropriate follow-ups.
Respond to and manage clinical emergencies within the practice.
Engage in safeguarding matters in accordance with local policy.
Collaborate with external agencies for the management of vulnerable patients.
Additional Responsibilities
Participate in audit, significant event analysis, and clinical risk reviews.
Support colleagues through mentoring and clinical guidance when required.
Contribute to the review and development of clinical policies and procedures.
Engage with local health initiatives and quality improvement programmes.
