Job summary

An exciting opportunity has arisen as Mayfield Surgery is looking for an enthusiastic and experienced Practice Nurse to work full time alongside our current GP and Nursing Team.

Main duties of the job

  • Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
  • Running asthma, COPD and diabetes clinics and other chronic disease management reviews.

About us

We are a practice located in the East of Northampton serving a population of around 9000 patients. The practice has a friendly, supportive team who work closely.

This consists of 4 GP Partners, 2 Salaried GP, 1 Advanced Nurse Practitioners, 1 Nurse Prescriber, 1 Practice Nurse, 1 Nurse Associate, 2 Managers and a team of 12 administrators/receptionists .

We use Systmone/ TPP and experience using this system would be an advantage, but full training will be offered. We offer a competitive salary depending on experience along with 25 days holidays plus bank holidays which rises after continued service.

Job responsibilities

Undertake, record and follow guidelines for the tasks for which you have received appropriate training and achieved competence;

Prepare and maintain environments and equipment before, during and after patient care, procedure, intervention.

Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being. Support patients to adopt health promotion strategies in healthy living.

Assess, plan, implement and evaluate individual treatment plans for patients with long term conditions.

Work with other health professionals to diagnose, monitor, manage and treat patients with long term conditions.

Support adherence for the recall of patients with long term conditions and adherence to prescribed medication.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing conditions in line with the National service Frameworks.

Participate in and promote the national immunisation programmes for children and adults with appropriate training.

Administer Adult/Child vaccinations under agreed PGD/PSD directives

Participate and support patients attending for travel vaccination, with appropriate training.

Perform opportunistic and planned NHS health screening checks.

Support patients in general health promotion, healthy lifestyle and promote self-well-being with appropriate referral as indicated.

Participate with QoF, enhanced services and medicines management initiatives to maintain practice high achievements in these areas.

Undertake appropriate activities/responsibilities related to own lead areas.

Support the PCN initiative.

Support and participate in shared learning across practice and wider organisation.

Provide support and guidance to the health care assistants and junior staff as needed.

Support the Nurse Manager in her role on behalf of the Partners/Managers.
