Job summary

At Rayleigh and District Primary Care Network (PCN), were transforming the way we deliver healthcare services to a population of 55,000 people across six member practices. As we continue to embrace new digital solutions, we are looking for an enthusiastic and experienced Digital Project Manager to join our team.

As Digital Project Manager, youll play a key role in driving our digital strategy forward. Youll lead several exciting projects such as cloud-based telephony, NHS App implementation, and patient triage systems. Youll work closely with local patients and practices to ensure the successful adoption of new technologies that improve patient care and practice efficiency.

Main duties of the job

Key Responsibilities:

Lead and manage digital transformation projects, ensuring they meet deadlines and goals.

Support the adoption of new technologies to enhance patient access and experience.

Collaborate with the Integrated Care Board (ICB) Digital Team and local practices to align digital initiatives with national and system strategies.

Use data to drive improvements in patient access, care quality, and operational efficiency.

Help practices within the PCN work together to deliver better care through digital tools.

Manage the overall transformation plan, ensuring resource allocation and risk management are in place.

This role is for a Digital Project Manager to work within the PCNs on a range of digital transformation projects such as the use of cloud based telephony, digital triage, online signposting and promotion of the NHS Apps.

The post-holder will report to the Transformation Manager and work closely with the Integrated Care Board (ICB) Digital Transformation team, which is responsible for setting the digital strategy and procuring and managing digital tools in primary care.

The ideal candidate will have a strong understanding of digital healthcare systems, project management methodologies, and the ability to work collaboratively with clinical teams, member practices and other key stakeholders.

KEY DUTIES:

The post-holder will:

Lead, manage, and deliver a portfolio of digital transformation projects within the PCN, ensuring they are completed on time, within scope and on budget.

Develop project plans, timelines to monitor progress to ensure successful project delivery.

Manage project risks and issues and ensuring timely resolution.

Collect and analyse practices data relating to their cloud-based telephony systems and undertaking quality improvement initiatives to support member practices.

Work closely with member practices to understand their personal digital needs and support its implementation.

Working with the Digital and Communications Manager to promote the use of online sign-posting platforms and implementation of the NHS App.

Support practices in the integration and use of NHS Apps to improve patient engagement and care delivery.

Ensure that digital and operational transformation is embedded and aligned with system and national strategies, working closely with the ICB Digital Team

Support Review of and improve the PCNs digital maturity, identifying opportunities for digital optimisation within the PCN

Support the effective adoption of national and local initiatives, including integrated working at neighbourhood and place level to improve access to patient services.

Use data to:

Understand demand, capacity, and activity - helping to drive improvements in patient experience of access.

Support population health management, improvements in care quality and PCN operational efficiency.

Provide feedback to PCN leadership and member practices on the progress of current initiatives, capacity for change, improvement, and issues so that plans can be adapted in response.

  • Good computer literacy and working knowledge of Microsoft Office products to produce, maintain and distribute documents including reports and presentations.
  • Good understanding of creating spreadsheets in Microsoft Excel, including layout, presentation, and project tracking.
