We have an exciting opportunity for a progressive and enthusiastic Advanced Nurse Practitioner to join us on our journey towards achieving the best possible standards for our patients.
Main duties of the job
You will be a degree qualified Advanced Nurse Practitioner with relevant clinical skills and expertise to work autonomously. You must have relevant experience to be an independent nurse prescriber enabling you to independently assess, diagnose, treat and refer patients as appropriate, as well as manage your own patient lists. A specialist area of interest would be an advantage. We are supportive of training and development.

Springhead Health is a large GMS GP Practice, serving approximately 22,300 patients.
We adopt a team approach to patient care and endeavor to monitor the service provided to patients, to ensure that it meets current standards of excellence.
We occupy a purpose-built building with excellent road and rail links, and a nearby high-speed train service into St Pancras London.
We are currently using EMIS Web clinical system.

Job responsibilities
- Provide the choice of
direct access to an ANP, both in the Practice and over the telephone, for patients
registered at the surgery
- Make professionally
autonomous decisions for which he/she is accountable
- Provide a first point
of contact within the Practice for patients presenting with
undifferentiated, undiagnosed problems, making use of skills in history
taking, physical examination, problem-solving and clinical
decision-making, to establish a diagnosis and management plan
- Instigate necessary
invasive and non-invasive diagnostic tests or investigations and interpret
findings/reports
- As an independent
prescriber : to prescribe safe, effective and appropriate medication as
defined by current legislative framework
- Provide safe,
evidence-based, cost-effective, individualised patient care
- Offer a holistic
service to patients and their families, developing where appropriate an
on-going plan of care/support, with an emphasis on prevention and
self-care
- Promote health and
well-being through the use of health promotion, health education,
screening and therapeutic communication skills
- Refer patients
directly to other services/agencies as appropriate
- Take the Practice lead
in developing and delivering specialist primary care services, such as long term disease management
(diabetes, asthma, chronic obstructive pulmonary disease, coronary heart
disease and hypertension)
- Work with nursing,
medical and health care assistant colleagues to ensure that National
Service Frameworks (eg Coronary Heart Disease/ Older People/ Diabetes/ Mental
Health) are being delivered
- Work with
mutli-disciplinary team within the Practice, and within NHS England and
CCG to promote integrated and seamless pathways of care
- Contribute to the
practice achieving its quality targets to sustain the high standards of
patient care and service delivery
· Participate in identification of community health needs and develop
patient/family-centred strategies to address them

Help develop
and set up new patient services and participate in initiatives to improve
existing patient services
- Be aware of and
contribute to planning and delivery of practice-based commissioning

On a rotational basis, undertake
telephone triage and duty roles. During your assigned duty times you must be
onsite and available, however if you need to go off site you will need to
arrange cover with another GP onsite.
