Job summary

Tower Hamlets is launching a new programme to reduce health inequalities and prevent long-term conditions across the borough through a community-centred approach. The programme is jointly funded by the council's Public Health team and NHS North East London (NEL). Primary care teams will deliver it in partnership with Public Health, voluntary, community sector organisations, and local residents.

We are looking for motivated individuals with experience working in local communities to join our Tower Network PCN, our South-West Locality team and Public Health Teams, to co-develop community-centred approaches to the prevention of long-term conditions.

Main duties of the job

The role will involve working with community groups, residents, GPs and a range of stakeholders to improve specified health and wellbeing outcomes for communities.

Key to this work will be an ability to build strong, trusting partnerships with residents, agencies and services and to empower communities to identify and develop community capacity and resilience to address health inequalities.

About us

We are the single largest PCN covering a list size of over 90,000 patients in the London Borough of Tower Hamlets, which became a limited company in 2022 by merging the two neighbouring PCNs in the south west locality, consisting of 6 practices stretching from Wapping to towards Mile End. The six practices are Goodman's Field Medical Practice, Jubilee Street Practice, Wapping Group Practice, City Square Medical Group, Harford Health Centre and St Katharine's Dock Practice.

The South-West locality comprises a vibrant multicultural population of 32.9% white and 47.1% Asian / Asian British. Our deprivation average score is 31.4, which makes us one of the more deprived areas of London, with areas of historically poor neighbourhoods sitting side by side with areas of affluence. The growth rate at Goodmans Fields, for example, is at 26.7% yoy, in line with the redevelopment of the Aldgate area. We have a young and mobile population with an average age of 33, which is at odds with the prevalence of long-term conditions within our patient population.

The PCN is currently delivering various community projects such as the Health & Wellbeing Hub, Doctor Digital, and Young Peoples events and is further involved in other community events organised by other voluntary sectors and Public Health England in partnership with the local councillors. We are working closely with East End Network on other pilot projects to improve access and bring some services closer to our communities.

  1. Develop community-centred approaches to prevention of long-term conditions by working with community groups, residents and a range of stakeholders to shape preventive services and activities, increase participation in preventive activities and improve specified health and wellbeing outcomes; especially communities that are not currently engaging in/accessing preventive activities.
  2. Build strong, trusting partnerships with communities, agencies and services and empower communities to identify and develop community capacity and resilience to address health inequalities.

Specifically the post will:

  • Work with a range of stakeholders to implement asset-based community and stakeholder engagement and development for long term condition prevention (using CSEAD and Well Communities methodology)
  • Support the development and implementation of local planning based on the above
  • Support community-led participatory budgeting and action to address identified barriers and facilitators in line with local plans Collect data as required for the monitoring and evaluation of the programme.

Key responsibilities of this Post

1. Engage with residents and stakeholders in the locality

a.Work in partnership with local organisations, statutory services, voluntary agencies and residents, using a high level of relationship management to create strong and enduring partnerships around long term condition prevention and tackling health inequalities

b. Work with local communities to shape preventive services and activities, increase participation in preventive activities and strengthen their capacity to include and support people who are vulnerable and socially excluded/marginalised

c. Maintain the strong and productive relationships and partnerships with and between residents and local services

d. Manage and support the development of local resident health champions /volunteers, including reward and recognition provision

e. Promote equality and value the diversity of individuals

2.Implement an asset-based community development approach in the locality

a. Support the local delivery team to use the Community and Stakeholder Engagement, Assessment and co-Design (CSEAD) methodology to engage priority communities, map assets and identify priority areas for action

b. Participate in briefings, training and action learning about the CSEAD methodology

c. Support local volunteer health champions to strengthen project outreach in their communities

d. Support the coproduction of local action plans for each area; including shaping and further developing preventive services and activities

e. Plan, attend and co-ordinate programme meetings, workshops and events

f. Support the facilitation of workshops and events

3.Action planning and participatory budgeting

a. Support the implementation of initiatives against co-produced action plans using agreed methodologies

b. Administer, manage and monitor participatory budgets.

c. Collect specificed data/information and review progress quarterly and refresh the action plan annually.

4.Communication and community insight

a.Develop culturally appropriate, accessible and socio-economically appropriate communication materials as required

b. Develop and maintain a clear understanding of local community strengths, resources, connections, gaps and opportunities to develop creative solutions

c. Gather information, insight and intelligence about health inequalities and long-term conditions in local communities

d. Provide regular updates to the locality team and Portfolio programme officer/public health project manager.

5.Team working at locality/PCN/ borough level

a.Co-ordinate and support the local delivery team to implement the programme focusing on specified target groups.

b. Liaise with local community connectors, social prescribers and other similar non-clinical roles

c. Work with the Operational Group to ensure a common approach across the borough/programme and share learning

d. Work closely with public health programme managers and other local team member

e. Co-ordinate and minute local meetings as required.

6.Monitoring and evaluation

a. Ensure good quality data/information is collected at community level and proper monitoring records are maintained for all eligible persons in the local area through use of an endorsed data system, providing information and data for reporting purposes

b. Respond to requests for information.

c. Assist in the collation of statistical information in order to provide evidence that the community project plan is meeting identified outcomes

d. Monitor local programme progress against agreed outcomes and undertake quarterly reporting

e. Work closely with programme evaluators and respond promptly to requests from evaluators, as required.

7.Manage and organise own time and activities

a. Keep adequate records of the work undertaken as agreed with line manager

b. Alert line manager to any issues in work (including concerns about individuals or work in the community)

c. Take an active part in development of own knowledge and skills in community-based development

d. Seek advice and support as and when appropriate

e. Attend all relevant and required training and comply with Network arrangements for continuing professional development

f. Work flexibly as required including out of core hours, agreeing time off in lieu (TOIL) in advance with the line manager
