Job summary
We are seeking a motivated and compassionate Salaried GP to
join our dynamic Home Visiting Team on a fixed-term basis until March 2028.
This role is designed to provide enhanced, on-the-day capacity for patients
identified as urgently needing home-based medical intervention.
The role will involve working closely with our skilled
paramedics and will also involve working on proactive care plans for our most
frail patients.
Hours: Flexible, we are looking for 2-4 sessions with a
focus on morning home visits, ideally the sessions would be spread out across
the week which suit someone working one session a day fitting around other
commitments.
Main duties of the job
Conduct timely home visits, prioritising earlier in the day
to support urgent patient needs.
Collaborate closely with paramedics to provide seamless,
responsive care in the community.
Work proactively with our frailest patients to develop
advanced care plans, including comprehensive medication reviews.
Help prevent avoidable hospital admissions by delivering
effective interventions at home.
Contribute to reducing overnight hospital stays and
conveyance rates to emergency departments.
Improve system flow by alleviating pressures on local urgent
and emergency care pathways.
Deliver high-quality, patient-focused clinical care aligned
with best practice and PCN objectives.

- Must have a valid driving licence and access to reliable transport (Mileage expenses will be reimbursed in line with PCN policy)

- Strong communication skills and the ability to work collaboratively with paramedics and other healthcare professionals
- Commitment to improving patient outcomes and reducing hospital admissions.
- Adaptability and enthusiasm for a proactive, patient-centred approach.
- The successful candidate would be working in both EMIS and SystmOne, training will be provided if needed.
