Job summary

The Orchard Medical Centre is a dynamic, friendly teaching practice based on the outskirts of Bristol and we are looking to recruit an experienced Nurse Practitioner working 30 hours per week spread over 3 working days, to join our existing friendly team within a dynamic and progressive primary care setting. The role includes autonomously managing a list of patients. You will be responsible for history taking, assessment, diagnosis and treatment of routine and same day presentations. Pay is depending upon level of experience/qualifications within the area. The ideal candidate will have experience in chronic disease and sexual health. You will supported by a team of friendly ANPS and GP's.

The post must include working each friday until 6.30pm

This post is only suitable to an experienced nurse practitiner who either prescribes or would be interested in undertaking non-medical prescribing.

Main duties of the job

This role is a challenging and diverse role where no 2 days are the same. It involves autonomously managing patients on a routine or acute same day list. This will include minor illness presentations, skin complaints, contraception. We would also be keen to see anyone who has experience in either a long term health condition such as asthma/diabetes or sexual health. As we are a teaching practice we are committed to providing training for the right candidate. You will be working as part of a team of 3 nurse practitioner's with guidance of a friendly team of GPs

The successful applicant should be flexible. There may also be the opportunity to work additional hours.

Job description

Job responsibilities

To be responsible for the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The nurse practitioner will be responsible for a number of clinical areas such as minor illness, health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

The following are the core responsibilities of the nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and well-being programmes

b. Provide a triage service, actively managing or referring patients or treating patients within own scope of practice

c. Support the clinical team in the development of individual treatment plans for chronic disease patients

d. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

e. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE

f. Provide wound care (ulcer etc.) to patients

g. Request pathology services as necessary, processing and interpreting results as required

h. Refer patients directly to other services as appropriate

i. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

j. Maintain accurate clinical records in conjunction with extant legislation

k. Ensure read codes and templates are used effectively

l. Maintain chronic disease registers

m. Develop, implement and embed well woman clinics

n. Develop, implement and embed well man clinics

o. Chaperone patients where necessary

p. Assist GPs with minor surgery when required

q. Prioritise health issues and intervene appropriately

r. Support the team in dealing with clinical emergencies

s. Recognise, assess and refer patients presenting with mental health needs

t. Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways

u. Implement vaccination programmes for adults and children

v. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

a. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

b. Support the clinical team with all safeguarding matters, in accordance with local and national policies

c. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

Deliver opportunistic health promotion where appropriate

In addition to the primary responsibilities, the nurse practitioner may be requested to:

a. If an independent prescriber, prescribe in a safe an effective manner, in accordance with PGD and extant legislation

b. Act as the audit lead, effectively utilising the audit cycle

c. Monitor and ensure the safe storage, rotation and disposal of medicaments

d. Contribute to the implementation of an effective training programme for staff and students within the practice

e. Provide clinical supervision for the nursing team

f. Participate in local initiatives to enhance service delivery and patient care

g. Support and participate in shared learning within the practice

h. Continually review clinical practices, responding to national policies and initiatives where appropriate

i. Be aware of and contribute to the planning and implementation of CCG patient pathway initiatives

j. Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA).

k. Maintain professional registration and adhere to revalidation requirements
