Job summary

The purpose of this dual role is to work as a Care Coordinator/Admin Assistant within the Primary Care Network. Supporting the NNHIP Project, Care Coordinating Team and Prescription Hub, for our five constituent practices within Connexus PCN: Birchwood Medical Practice (Brislington), Nightingale Valley Practice (Brislington), Priory Surgery (Knowle), Stockwood Medical Centre (Stockwood) and Wells Road Surgery (Totterdown).

Main duties of the job

The role of the CareCoordinator/Admin Assistantin the Primary Care Network is to support our multi-disciplinary clinical team to ensure that our patients, particularly those who are vulnerable or have complex needs are able to access services which meet their needs.

About us

Connexus Primary Care Network brings together five GP Practices serving 50,000 patients in the Brislington, St Annes, Knowle, Totterdown and Stockwood areas of South Bristol. The PCN is located in South Bristol locality of BNSSG and employs a small staff team that is responsible for delivering a range of services under contract with NHS England.
