Job summary

This is an exciting opportunity for an experienced Advanced Physiotherapy Practitionerto develop their clinical practice in a leadership role.

You will work directly with a small team of First Contact Practitioners to patients across our Primary Care network of Practices in South Bristol.

You will need to have obtained or in the process of obtaining a Advanced Practice qualification (post-grad diploma or degree, or Masters if qualified after 2020). You should also hold or be working towards a prescribing qualification.

Main duties of the job

You will lead a small team of FCPs to maintain and develop high quality clinical services to our patients. Advanced clinical practice is a level of practice characterised by a high degree of autonomy and complex decision making. This must include education to masters degree level and having the capabilities of advanced clinical practice set out in section one of the Multi-professional Framework for Advanced Clinical Practice in England. This encompasses the four pillars of clinical practice, leadership and management, education and research.

Main responsibilities

Clinical

- Independently triage, assess, evaluate, and diagnose patients with highly complex disorders and possible multiple pathologies using advanced critical thinking, expert clinical and diagnostic reasoning skills including instigating appropriate clinical investigations and interpreting results to enable the development of immediate management plan which will meet the physical and psychological needs of the patient and family.

- Be able to justify clinically referrals to other specialist services. This will include referral to acute and specialist services such as surgical orthopaedics, rheumatology and neurology, other AHP services, nursing services and to the primary care medical teams. This requires advanced knowledge of the role of other specialist health professionals.

- An ability to recognise RED flags, serious pathology and potential underlying non-MSK related disease, that may present as MSK symptoms and to refer appropriately. When needed accelerate the patients referral to other health professionals/services.

- May initiate medication prescribing, administration and management in line with Patient Group Directives or Prescribing as required for specialist role and in line with prescribing competence. NMP qualification desirable but not essential.

- Maintain timely and accurate patient document records to reflect care provided and to be professionally and legally accountable.

- Work collaboratively with practice and PCN colleagues in a multidisciplinary team, maintain relationships with orthopaedic consultant teams, relevant MDT and physiotherapy services in community and acute settings. Where appropriate develop relationships with wider health and social care agencies, for example mental health teams, local authorities, third sector providers and patient groups, provide specialist advice to healthcare professionals, other agencies, carers, clients and relatives.

- Actively takes a personalised care and population centred care approach to enable shared decision making with the presenting person. Actively engages in care from a Population care viewpoint.

Service Development & Education

- Take a key role in leading and developing the service to improve practice and work collaboratively, implementing and evaluating evidence based standards, guidelines and policies.

- To identify and be actively involved in audit and research activity which will contribute to the development of the service. Flexible skill set to adapt to and meet needs of the PCN Population and support public health.

- To plan and lead specific areas of PCN/practice developments and including implementing best practice and management in order to enhance the patient journey.

- To promote a culture of continuous learning and improvement within the service and the team; identifying opportunities for service development.

- To support/facilitate education and training within the PCN

- To provide supervision and support to the members of the FCP team

Research and Audit

- To initiate and contribute to research in specialist field which will directly contribute to patient management and also the improvement and development of the service.

- Interpret newly generated research findings in specialist field determining how to implement through application of advanced knowledge and skills, disseminating findings internally to influence best practice.

- Develop and undertake regular audits which promote and facilitate service improvement.

Leadership

- The postholder will establish strong working relationships with Physiotherapist and management of staff to encourage good clinical practice.

- The postholder will work with the GP leads of the five Practices to identify and follow up opportunities for the improvement of care.

- The postholder will provide clinical support and advice to colleagues including Care Co-ordinators and Physiotherapist.
