PRACTICE NURSE
Due to
increased demand, we are looking for a Practice Nurse to join our friendly
Practice family. Oswald Medical Centre
is an award-winning Practice serving over 16,000 patients. We are based across 3 sites in Accrington,
Oswaldtwistle and Blackburn. We pride
ourselves on our approach to providing caring services and offer a range of
standard NHS and enhanced services including minor surgery.
We have an extensive clinical team comprising of: 4 GP Partners, 1 Salaried GP, 3 Advanced Nurse Practitioners, 3 Practice Nurses, 1 Nurse Associate and 2 Healthcare Assistants.Supporting our Clinical Team is a large Administrative Team, including a dedicated Pharmacy Technician who heads up our in-house Prescribing Hub, a Physiotherapist, Health & Wellbeing Coach, Social Prescriber.

Main duties of the job
Main Duties of the Role of Practice Nurse - Primary Care
To provide a high quality care service based on the needs of the Practice Population which includes, treatment, screening, preventative care and patient education
- To manage patient with Long Term Conditions
- Participate in National Screening - Cervical Cytology & Baby Vaccinations
- Select, Carry out and interpret results of specific tests
- Provide Travel health advice and administer appropriate vaccinations
- Audit
- Present and contribute to Clinical Team Meetings
- To monitor and work on QOF areas and QF
- Seasonal Vaccinations - Flu, Pneumococcal, Shingrix, RSV

Duties and
Responsibilities - Practice Nurse
Clinical
1.
To manage patients with chronic disease, running specific
clinic sessions and supporting the maintenance of robust call and recall
systems
2.
Participate in national screening work such as cervical
cytology and deliver the national immunisation and vaccination schedule
3.
Provide travel health advice and administer appropriate
vaccinations
4.
Treat patients with minor injuries
5.
Provide directly and assist GPs with contraceptive
services
6.
Break bad news to patients/parents/carers and provide
appropriate ongoing support to meet identified needs
7.
Act as chaperone when required
8.
Undertake health promotion to help educate patients
9.
Select, carry out and interpret results of specific tests
10.
Undertake anticoagulation clinics to include
interpretation of blood tests and dosing
Administration
11.
To maintain accurate patient records and enter onto the
computer using agreed Snomed Codes
12.
Ensure accurate completion of all necessary documentation
associated with patient health care
13.
To collect data and participate in regular Practice audit
and research projects
14.
To contribute to the preparation of any Practice
development plans
General
15.
To maintain own annual registration with the NMC and
adhere to Code of Professional Conduct
16.
To maintain a safe and therapeutic environment for all staff,
patients, carers and co-workers from other agencies. Ensure all statutory
regulations (e.g. Health & Safety, COSHH, etc) are implemented and
adhered to
17.
To promote a professional approach at all times by attire
and attitude to patients and staff both in and out of the Practice
18.
To maintain own continuous professional development and
ensure PDP kept up-to-date in line with the requirements of the Practice
19.
Attend and participate in staff and Practice meetings and
in-house clinical training sessions
20.
Any other reasonable duties that may be required from time-to-time
