Job summary
St John's Medical Centre has an exciting opportunity for a Practice Nurse to join our enthusiastic and supportive Practice Team. The successful candidate must be organised, motivated and caring to ensure the highest levels of care are delivered to our patient population through patient assessments, diagnosis and clinical management.
The role is for 30 hours a week working over 4 days including: Monday, Tuesday, Wednesday and Friday. Pay is dependant on experience.
Main duties of the job
We are seeking a dedicated and fully qualified Practice Nurse to join our dynamic primary care team. The successful candidate will deliver high-quality, evidence-based nursing care to patients in a general practice setting, supporting the management of long-term conditions, health promotion and preventative care. This role also includes home visits for chronic disease management and phlebotomy.

St John's Medical Centre is a large, fast paced GP surgery located in Grantham, offering excellent transport links with easy assess to the A1 and on the mainline for London. Two notable Grammar schools in Grantham are Kesteven and Grantham Girls School and The Kings School. We currently have circa 13,000 patients registered at the Surgery and retained our 'Good' rating in our latest CQC Regulatory Review. We are a Training Practice and regularly welcome GP Registrars, Nursing Placements and Work experience from Colleges/ Schools in the local area.

Duties and Areas of Responsibility
- Provide nursing care and support to patients across all age groups.
- Undertake chronic disease management (e.g. Diabetes, Asthma, COPD and Hypertension.)
- Administer vaccinations and immunisations, including childhood and travel vaccines.
- Maintain and observe the cold chain for all vaccines and temperature sensitive medications.
- Perform cervical cytology (smear tests) in line with national screening guideline.
- Conduct wound care, dressings and suture removal.
- Carry out NHS health checks and lifestyle advice (e.g. smoking cessation and weight management.)
- Support patients with family planning and sexual health advice.
- Assist with minor illness clinics and triage where appropriate.
- Undertake home visits for chronic disease reviews and phlebotomy as required.
- Maintain accurate and up-to-date clinical records using SystmOne.
- Participate in clinical audits, quality improvement initiative and multidisciplinary meetings.
- Adhere to infection control policies and contribute to safeguarding procedures.
- Offer support and flexibility to cover colleagues' annual leave and sickness absence when needed.
