Job summary

St John's Medical Centre has an exciting opportunity for a Practice Nurse to join our enthusiastic and supportive Practice Team. The successful candidate must be organised, motivated and caring to ensure the highest levels of care are delivered to our patient population through patient assessments, diagnosis and clinical management.

St John's Medical Centre is a large, fast paced GP surgery located in Grantham, offering excellent transport links with easy assess to the A1 and on the mainline for London. Two notable Grammar schools in Grantham are Kesteven and Grantham Girls School and The Kings School. We currently have circa 13,000 patients registered at the Surgery and retained our 'Good' rating in our latest CQC Regulatory Review.

The role is for 30 hours a week and includes working: Monday, Tuesday, Wednesday and Friday. Pay is dependant on experience.

Main duties of the job

To be responsible for the delivery of practice nursing services, working as part of the practices multidisciplinary team who provide care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well woman and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

Primary Duties and Areas of Responsibility

  • To liaise with the registered GP and other practice based staff in addition to all other providers and services utilising, where appropriate a multi-disciplinary approach
  • To implement and review individual care plans, self-management plans ensuring preventative measures are detailed and agree trigger thresholds to contact the surgery
  • Carrying out home-visits to patients where an urgent clinical need has been identified including those esculated by EMAS or a member of the Neighbourhood Team
  • Liaise with primary, secondary and specialist care services as required
  • Work with the Neighbourhood Team to help identify patients at risk of loss of independence or admission to hospital as a result of inadequate social support
  • Act as a point of contact between GP, Neighbourhood Team, people and their carers
  • Ensure care plans are communicated to GPs and other members of the Neighbourhood Team involved in the patients care
  • Maintain accurate and up to date records of patients consultations using relevant software e.g Systm One
  • Complete relevant health checks in line with patient needs
  • The ability to work alone- when undertaking home visits/ work in the community
  • Develop, implement and embed health promotion and well-being programmes
