Job responsibilities

Clinical Responsibilities

Undertake long term condition reviews including hypertension, diabetes, CHD and heart failure.

Support the nursing team by delivering health promotion programmes.

Carry out baseline observations such as pulse oximetry, blood pressure, temperature, and pulse rate, recording findings accurately on SystmOne.

Carrying out urine testing.

Facilitate routine and 24-hour BP monitoring, advising patients accordingly.

Undertake wound care, dressings and other clinical tasks.

Carry out BMI checks (weight/ height) and recording these accurately.

Act as a chaperone when required.

Undertake phlebotomy for adults and some children as appropriate to training, experience and competency and send to the laboratory.

Administer both adult flu vaccines and childrens nasal flu under relevant PSD

Removal of suture and clips

Administer B12/ Pneumonia/ Shingles vaccinations.

Carry out ECGs/24-hour ECGs ensuring results are available for GP.

Conduct home visits for housebound patients.

Deliver opportunistic health promotion where appropriate, including smoking, weight management and exercise programmes.

Carry out INR testing, if had relevant training.

Deliver opportunistic health promotion where appropriate.

Ensure specimens are recorded accurately and ready for onward transportation and on occasion take samples to pathology at Grantham Hospital.

Provide support during minor operations/ gynaecology clinics as required.

Ensure all clinical rooms are adequately stocked and prepared for each session.

Ensure fridges are cleaned routinely in accordance with extant guidance.

Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the organisations IPC policy.

Team Working

To work appropriately with other members of the nursing team to ensure good patient care, adherence to protocols and procedures and implementation of change when necessary.

To attend Practice meetings as appropriate.

To work with other team members to ensure the implementation of the Quality & Outcome Framework.

Service Delivery

To support the smooth operation of the nursing team. This will include providing flexibility to ensure that holidays, study leave, and sickness are covered

To assist in providing cover for HUB shifts when required.

To ensure the correct use of medical equipment, ensuring any problems are reported and requesting training when needed.

Take on responsibilities as delegated by the Nurse Lead, these may include but are not limited to; Health & Safety tasks, stock checks, checking and reviewing of medical equipment in rooms, ordering and maintaining stocks of drugs and supplies and any other appropriate tasks.

To participate in Clinical Governance activities and engage with educational opportunities.

To participate in the planning an implementation of change within own area of responsibility.

To participate in any ongoing training required for the role and ensure all mandatory training is completed.

Person Specification

Qualifications

Essential

• Qualified in phlebotomy

Desirable

• BTEC/ NVQ- Health and Social Care- Level 2 or 3

Additional Requirements

Essential

• Flexibility to work both core and outside of core hours to ensure practice also has adequate cover
• Access to own car/ transport
• Disclosure Barring Service (DBS) enhanced check.

Experience

Essential

• Experience of working as a Health Care Assistant
• Experience of undertaking phlebotomy both adults and some children
• Administer B12, Flu (Adult/ Children- Nasal), Pneumonia and Shingles vaccinations.
• Experience in wound care including dressings.
• Experience of working automatously and as part of a team

Desirable

• Computer Skills (SystmOne / Accurx preferred- however full training will be provided)
• Experience of working in Primary Care/ Hospital setting
• Experience of Chronic Disease Management
• Experience of carrying out INR testing
• Understanding of Quality and Outcome Framework/ IIF targets
