Primary Care Network Care Co-Ordinator

Job summary

The PCN Care Co-ordinator supports the delivery of personalised, coordinated care for patients across 10 GP practices serving a population of over 100,000 patients.

The post holder works closely with GPs, nurses, pharmacists, social prescribing link workers, health coaches, and community services to identify and support patients with long-term conditions, complex needs, frailty, and social vulnerability.

The role acts as a central point of contact for patients and professionals, helping individuals navigate health and care systems to improve outcomes and reduce health inequalities, while also supporting operational delivery across the Network.

The role is based at Park Medical Practice, Cottam Lane Surgery which is a member practice of the Primary Care Network

Manage and coordinate a caseload of patients, particularly those with long-term, complex or frailty-related needs.

Support multidisciplinary team meetings, including preparation, attendance, and follow-up actions.

Coordinate care across GP practices, community services, mental health teams, drug and alcohol services, and voluntary sector organisations.

Promote person-centred care and shared decision-making.

Support patients to access self-management education, peer support, and health coaching services.

Provide administrative support across 10 GP practices serving over 100,000 patients.

Create and manage Enhanced Access rotas, coordinating clinicians and support staff to deliver out-of-hours and extended access services, ensuring safe and timely patient care.

Liaise with pharmaceutical companies and coordinate the ordering and distribution of COVID-19 vaccinations across all practices.

Monitor performance data and support reporting and quality improvement initiatives.

Organise professional meetings and produce accurate, timely minutes.

Plan and deliver patient health and wellbeing events.

Support healthcare improvement projects, including frailty pathways and homeless health initiatives.

Organise GP and nurse clinics within homeless shelters, including stock ordering and coordination.

Manage allocated project budgets in line with governance and financial procedures.

About us

Greater Preston Primary Care Network is part of Heart of Lancashire Federation and covers 10 GP practices in the Preston Locality. The PCN has responsibility for Enhanced Access Provision, Directed Enhanced Service Contract, local initiatives that support the delivery of patients registered to the PCN, work with the Cancer alliance, rough sleepers and those patients residing in areas of low uptake of screening.

This is an exciting opportunity to join a forward thinking PCN team who strive to develop enhanced services for its registered population. The PCN team has a motivated Clinical Director, and three managers along with carecoordinators and social prescribers.
