Our Frailty Service has gone from strength to strength, supporting Brownlows housebound population to achieve better health outcomes and wellbeing. We were proud to be shortlisted for Team of the Year in the 2024 GP Awards, and our Nursing team was awarded Nursing Team of the Year. We are now expanding our dedicated team and looking for a passionate professional to join us.
Role Overview:
As a key member of the Brownlow Health Frailty Team, you will spearhead the development of a new Acute Home Visiting Service, complementing our existing services. This innovative service will be supported by a multi-disciplinary team.
Main duties of the job
Key responsibilities will include:
- Acute and planned home visits for patients within the local Brownlow Health housebound population. These will typically be moderately/severely frail, living with Dementia or homeless, as well as refugee and Asylum seekers.
- You will have experience in performing a holistic assessment and managing both acute health issues and long term conditions, referring patients to admission prevention services or secondary care as necessary whilst also collaborating with external services, patients and their families to create comprehensive care plans and follow-ups.
