Job summary
We are recruiting to our homeless and complex
needs team to provide outreach services to patients in hostels and in practice who require support with
their health needs, we are an award winning team that comprises of GPs and
Nurses who provide holistic care to this complex group.
Main duties of the job
To provide long term condition
management
Ensure all vaccinations and
immunisations are up to date and assess the patients risks and vaccinate
appropriately
Assessments in hostels and in Practice
at HAC (homeless access clinic)
Liaise with other agencies to ensure
we are providing the best outcomes for this client group

The Homeless Nurse will work within a
multi-disciplinary team to deliver high quality care and support the individuals
experiencing homelessness. The role involves providing health assessments,
support with managing complex health conditions and liaising with other
services to ensure a holistic approach to care both in the primary care setting
or the hotels they reside in.
Primary Key Responsibilities:
Conduct health assessments and provide a person- centered care for
the individual who are homeless or at risk of homelessness.
Administer
treatments, medication, immunisations as required, including wound care,
infection control, management of long term medical conditions and support
around mental health, substance misuse and other complex health issues.
Secondary Responsibilities:
Liaise with outside agencies to support the patient with their health and
social needs.
Act as an advocate for their individual health needs.
Participate in supporting the practice with GP and Nurse trainees as

Attend meetings and conferences relevant to the role.
Participate in monthly educational meetings.
Undertake mandatory training and educational training to ensure
delivering safe, effective and up to date care.
